administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle tag#CBZ5363, VIN# correct per the firm application, capacity 2,000 gallons. The tank
appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The
lettering is correct on both sides of this vehicle except that the phone number displayed on both permitted
vehicles is 704-626-2699 (which is the number listed on last year’s application) – however the application
recently submitted shows a phone number of 704-245-7191. Rooter Express needs to correct the firm
application to reflect the phone number listed on the vehicles or the lettering (for the phone number) on
the two vehicles needs to be corrected.
2. Reviewed the truck logs at this time. The disposal location for each pump job was missing. Reviewed the
information that shall be maintained for each septage pump job – ensure that all required information is
being recorded including the disposal locations.
