OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1) Pumper vehicle(s) inspected: TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) ZB-54462 1HTMMAAN95H106741 2,500 2) The firm name, town name, phone number, and firm permit number were on both sides of the pumper vehicle. The letters were three (3) inches or greater in height. The lettering was distinguishable and not blocked from view. The lettering on the pumper vehicle meets the requirements of 15A NCAC 13B .0835(b). 3) The tank on the pumper vehicle was made of steel and affixed to the frame of the truck as per 15A NCAC 13B .0835(a)(1). 4) The valves appeared to be in working order and closed as per 15A NCAC 13B .0835(a)(2). Caps present on hose ports. 5) Access ports on tanks appeared to be in good repair and sealed as per 15A NCAC 13B .0835(a)(3). 6) No leaks noticed as per 15A NCAC 13B .0835(a). 7) The pumper vehicle passes inspection. A disposal authorization must be submitted before a permit can be issued. Once you have a disposal authorization, please email me a copy of the authorization. Cobb, Environmental Program Consultant Division of Waste Management, NCDEQ Regional Representative Sent on: X Email Hand delivery US Mail Certified No. [ _] SEPTAGE PUMPING LOG INSTRUCTIONS The NC Septage Management Rules (15A NCAC 13B .0839(a)) and the Federal Rules (40 CFR 503.17(b)) require that specific information be recorded and maintained for septage management firms. Incomplete record keeping may result in penalties being issued by State and/or Federal agencies. In order to assist you in maintaining the required records, the attached form has been prepared as a guide. The form contains the information that septage management firms are required to keep. The federal rules are located in 40 CFR 503.17(b), which can be found in the Appendix I of the North Carolina Septage Management Rules. You are not required to use the form. It is only an example to assist you in keeping the records required by state and federal rules. The log should be utilized to record each pumping event for septic tanks and grease traps. Portable toilet pumpers may record route totals within each town rather than each pumping event if desired. Section I: Write in your firm name and your firm permit number in the spaces provided. Section II: Column 1: the date that the septage was pumped into your truck. Column 2: the type of septage that you pumped. The most common types are domestic septage, portable toilet waste and grease. Column 3: Amount of septage pumped in gallons. Column 4: Location of septage pumped. (An address- i.e. 401 Oberlin Rd. Raleigh) Portable Toilet Waste: Each route should include at minimum the town name where the portable toilet waste is pumped from. Column 5: The date you pumped the septage out of your truck. Column 6: where you put the septage you removed from your truck. Write in the SLAS permit number if land applied, “WWTP” with plant name if taken to a wastewater treatment plant, SDTF permit number if put in a storage tank or treatment facility, or permit number for the solid waste facility if taken to a compost facility or landfill.
