McKenney, VA 23872 GPS COORDINATES: N: NA E: NA FACILITY CONTACT INFORMATION: Name: Mr. Tommy Edmunds Address: Same as above Telephone: 804-478-4688 Email address: Tommy@edmundswasteremoval.com PARTICIPANTS: Chester Cobb (DWM) and Tommy Edmunds STATUS OF PERMIT: Pending (Permit issued on 02/03/2021) EXPIRES: PURPOSE OF SITE VISIT: Pre-operational STATUS OF PAST NOTED VIOLATIONS: On January 14, 2021, a Notice of Violation was issued for violations to 15A NCAC 13B .0832(b)(3), 15A NCAC 13B .0832(b)(2), and NCGS 130A-291.1(c) for the placement of portable toilets at a construction site off of Peete Farm Road in Warren County. Received email with Invoice attached documenting the removal of wastewater from the portable toilets by a permitted firm. The portable toilets were removed afterwards. The last step of the compliance is to obtain a firm permit. OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1) Pumper vehicle(s) inspected: TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) UA73-238 3C7WRMBLXKG555084 900 WASTE / 400 WATER 2) The firm name, town name, phone number, and firm permit number were on both sides of the pumper vehicle. The letters were three (3) inches or greater in height. The lettering was distinguishable and not blocked from view. The lettering on the pumper vehicle meets the requirements of 15A NCAC 13B .0835(b). Mr. Edmunds plans on enlarging the phone number on the cab doors to be at least 3 inches in height. Pictures texted on 02/09/2021. See pictures of the pump truck below. 3) The tank on the pumper vehicle was made of steel and affixed to the frame of the truck as per 15A NCAC 13B .0835(a)(1). 4) The valves appeared to be in working order and closed as per 15A NCAC 13B .0835(a)(2). Caps present on hose ports. 5) Access ports on tanks appeared to be in good repair and sealed as per 15A NCAC 13B .0835(a)(3). 6) No leaks noticed as per 15A NCAC 13B .0835(a). 7) The pumper vehicle passes inspection. A 2021 Septage Management Firm Permit was issued on 02/03/2021. Driver side of pump truck. Passenger side of pump truck.
