STATUS OF PAST NOTED VIOLATIONS:
Violations of North Carolina General Statute 130A-291.1 (c); 15A North Carolina Administrative Code (NCAC) 13B .0832(b)(2) and 15A NCAC 13B .0832(b)(3) have been corrected by removal of portable toilets from service and ceased
operation. A compliance order is in process.

ADDITIONAL COMMENTS
1. The inspection was conducted at the Asheville Regional Office, Swannanoa in Buncombe County, North Carolina.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

2. The following firm truck was inspected:

Photos taken by Troy Harrison on 12/17/2018 3. The tank appeared to be structurally sound and bolted to the frame.
4. Valves and access ports appeared to be secure. Caps were present on the access ports.
5. The truck was properly marked in accordance with the lettering requirements. 6. No leaks were observed.
7. The tank was on a slide in unit on the vehicle and will need to be permanently attached to the truck bed
or frame of the vehicle before use. When this has been completed, a re-inspection will be required. 8. A reminder, as a new septage management firm in North Carolina, this vehicle cannot be used until a
permit is issued by the Solid Waste Section.
