ADDITIONAL COMMENTS
1. The inspection was in Mills River, Henderson County, North Carolina. 2. The following firm truck was inspected:

3. The tank appeared to be structurally sound and bolted to the frame. 4. Valves and access ports appeared to be secure. Caps were present on the access ports. 5.
