STATUS OF PERMIT: Active Permit Issued: June 4, 2015 Expired: June 4, 2020* *Permit renewal submitted PURPOSE OF SITE VISIT: Permitting Inspection Pre-operation inspection of recently installed tanks STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. This facility currently consists of one 4,500-gallon, one 12,000-gallon, one 13,700-gallon, one 24,000-gallon and one 24,500-gallon tanks for the storage of treated and untreated domestic septage (including portable toilet waste) and grease trap pumpings. The five tanks have a total of 78,700 gallon capacity. The current permit modification was issued for the addition of de-watering boxes. 2. Two additional 12,000-gallon steel tanks have been installed with an additional screening box. One tank has been plumbed with plastic pipe. The other will utilities flexible non-permanent hoses. Soil has been used to secure the tanks in their location, however; additional soil should not be added as this is permitted as an above ground tank. New Receiving/Screening Box Additional Tanks 3. Appropriate signage was in place along the access road. 4. Odors were detected around the facility but not off site. 5. It appeared that the setback requirements were being maintained. 6. All tanks at the facility appeared to be structurally sound and not leaking. Access around the tanks has been maintained to allow for inspection. 7. Vectors were not present at the time of inspection nor were objectionable odors observed. 8. The operational logs were not reviewed.
