(includes legacy URS and herein referred to as AECOM) is pleased to provide the attached Analytical Data Table Report for recently completed monitoring well sampling and well housing modifications at the T&J Cleaners facility (site) located at 2423 Crabtree Boulevard, Raleigh, Wake County, North Carolina. This work was completed in accordance with the scope of work documented in an AECOM Assessment Cost Proposal, as approved by DSCA under State Lead Authorization for Work (SLAW) 021B on July 2, 2019. On May 29, 2019, AECOM was informed by Darin McClure of Mid -Atlantic Associates, Inc. (Mid - Atlantic), the environmental consultant for the owner of the 2423 Crabtree Boulevard property, that ongoing grading activities and parking lot modifications being completed as part of site redevelopment activities could impact the existing monitoring well network. After several phone calls to the grading contractor, AECOM met the contractor onsite on June 14, 2019, to discuss the proposed activities and what impact they would have on existing monitoring wells. While onsite, AECOM located four monitoring wells in the parking lot (MW-5, MW-12, MW-13, and DMW-18), but could not locate monitoring wells MW-6, MW-7, and MW-15, as it was evident that some construction activities and grading had already occurred in the areas where these monitoring wells were located. AECOM provided the grading contractor a copy of the monitoring well location map (included as Attachment 4), as well as GPS coordinates for the missing monitoring wells so they could attempt to locate the wells during the next phase of grading activities. The planned grading activities were slated for the week of July 8, 2019 and the grading contractor asked for the site monitoring wells located in the parking lot to be raised by at least 6 inches to accommodate new site grading elevations. AECOM and DSCA discussed the planned site grading activities (i.e., scope and schedule) and the likelihood that the missing monitoring wells would get, or already had been damaged/destroyed, as part of the site improvements. As these site monitoring wells have been sampled at least eight times over the last decade and the chlorinated solvent contaminant plume appears to be stable, optimization to the monitoring well network was proposed. Therefore, costs were provided for a licensed North Carolina driller to mobilize to the site and abandon monitoring wells MW-6, MW-7, and MW-15, if located, and modify monitoring wells MW-5, MW-12, MW-13, and DMW-18. Prior to well abandonment activities, AECOM proposed the collection of groundwater samples from all site monitoring wells for analysis of total manganese and total iron, as these constituents have not been monitored as frequently as the dissolved phase volatile organic compounds (VOC) at the site and their presence, magnitude, and distribution are important in evaluating secondary impacts to groundwater AECOM Mr. Billy Meyer T&J Cleaners — DC920021 October 3, 2019 Page 2 of 2 quality attributed to a potassium permanganate injection completed in 2015, as part of the site remediation activities. On June 26, 2019, AECOM deployed HydraSleeves in all site monitoring wells, except for monitoring wells MW-6, MW-7, MW-15, MW-16, and DMW-18, which could not be located due to parking lot construction activities. On June 28, 2019, AECOM returned to the site to complete the comprehensive groundwater gauging and sampling event for iron and manganese via the HydraSleeves previously deployed. As shown on Attachment 7, elevated iron and manganese concentrations are prevalent across the site, including upgradient monitoring wells MW-3 and MW-11, which should not have been affected by the potassium permanganate injection. Manganese and iron concentrations were the most elevated at the potassium permanganate injection area near MW-2 in the rear of the former drycleaner at 521,000 micrograms per liter (ug/L) and 105,000 ug/L, respectively. `Background" manganese concentrations at upgradient monitoring wells MW-3 and MW-I I were 55.7 and 145 ug/L, respectively, which is higher than the NC 2L Groundwater Quality Standard of 50 ug/L for manganese. Site concentrations of manganese ranged from 18.4 to 2,040 ug/L and iron concentrations ranged from 562 to 58,400 ug/L in downgradient/cross gradient shallow monitoring wells, as shown on Attachment 7. Additionally, on July 1, 2019, AECOM oversaw well repair activities by a licensed North Carolina driller of site monitoring wells MW-1R, MW-5, MW-12, MW-13, and DMW-18 to match the proposed grade of the new parking lot. Monitoring wells MW-6, MW-7, MW-15, and MW-16 are assumed to have been destroyed by parking lot renovations and could not be located for proper abandonment by a licensed driller. Documentation of the wells before and after the repairs can be found in the attached photo log, included as Attachment 17. November 2002 DL Restoration, LLC conducted a Phase I Environmental Site Assessment (ESA) at Gateway Plaza and reported stained soils in the courtyard behind the dry cleaner and noted the presence of several drums. Ed Aguirre & Associates, Inc. (Aguirre) conducted a Limited Soil & GW Assessment, which included the advancement of December 2002 five soil borings and the installation and sampling of a temporary well in the vicinity of the soil boring that had the highest PID detection. March 2003 Meredith College completes DSCA Petition Questionaire. Earth Tech installed three Type II monitoring wells (MW-1, MW-2, MW-3) and sampled MW-2 and MW-3 for VOC November 2003 analysis by SW-846 826013. A groundwater sample was not collected at MW-1 due to the well being dry. Earth Tech also collected and submitted soil samples at each well for VOC analysis. December 2003 T&J Cleaners is petitioned into the DSCA program. December 6 & 7, MacTec abandoned MW-1 and installed three Type II shallow monitoring wells (MW-1R, MW-4, MW-5). All site wells 2005 (MW-lR to MW-5) are sampled and submitted to the laboratory for VOC analysis. MacTec advanced 14 soil borings in the courtyard, three borings in front of store, and collected samples for VOC June 7-9, 2006 analysis. MacTec also installed/sampled six Type II shallow monitoring wells (MW-6 to MW-12). Three surface water samples were collected and submitted for VOC analysis (SW-1 to SW-3). January 9 to 11, 2007 MacTec installed/sampled two Type II shallow monitoring wells (MW-13, MW-14) for VOC analysis. MacTec also sampled four surface water samples (SW-1 to SW-4). March 2008 MacTec conducted pre -injection sampling at MW-1R to MW-14 for VOC analysis. MacTec installed one Type II shallow monitoring well, MW-15, and collected SW samples. April 18, 2008 MacTec resampled MW-5, MW-6, and MW-15 due to abnormal laboratory results. April 24, 2008 MacTec injected Regenesis HRC A into four injections points near MW-15. May 2008 MacTec sampled MW-15 for total organic carbon (TOC), methane, ethane, ethene (MEE), sulfate, nitrate, nitrite, iron, and manganese. June 2008 MacTec submited Injection Event Record to NCDENR Aquifer Protection Section and sampled MW-15 for TOC, MEE, sulfate, nitrate, nitrite, iron, and manganese. January 18 to 27, 2010 URS installed two shallow Type II wells (MW-16 & MW-17) and one Type III well (DMW-18), advanced 12 soil borings, collected four surface water samples, six Henry Push Point samples, two soil gas samples, one subslab sample, and completed sitewide GW gauging/sampling/survey. July 1, 2010 URS collected five indoor air samples from adjacent tenant spaces and one ambient air sample. July 6, 2010 URS collected five subslab samples from adjacent tenant spaces. February 4 & 5, 2011 URS completed fugitive emissions study in T&J Cleaners and two adjacent tenant spaces. May 2012 T&J Cleaners vacates tenant space. August 7 & 8, 2012 URS collected four indoor air samples from adjacent tenant spaces and former dry cleaner. URS collects six additional soil samples (S13-1 to S13-6) to further delienate soil contamination. November 9 & 13, URS completes indoor air and sub -slab vapor sampling at Arrow Pawn. 2012 December 12 & 13, URS completes site -wide gauging event, samples select monitoring wells, collects surface water samples, collects indoor 2012 air sample at former dry cleaner, and attempts to collect an off -site soil gas sample. December 21, 2012 URS collects a soil gas sample at 2424 Crabtree Boulevard. March 29, 2013 URS completes indoor air sampling at former dry cleaner and three adjacent tenant spaces. URS attempts the installation of five V temporary well points (TW-1 to TW-5) around the source area in the courtyard to April 22 & 23, 2013 evaluate the extent of grossly contaminated perched water. Groundwater samples were collected from three well points and soil samples were collected from all well points. July 14-16, 2015 URS oversees the installation of three horizontal subslab depressurization (SSD) system lines beneath the rear of the former drycleaner. July 20-23, 2015 URS oversees injection of liquid potassium permanganate in the courtyard area. Daylighting occurs in the middle of the courtyard. A total of 1,052.5 gallons of injectate slung containing 1,559 pounds of potassium permanganate are utilized. September 17-19, URS oversees application of granular potassium permanganate in the courtyard area via surficial trenching and borehole 2015 advancement to 5 feet below ground surface. A total of 2,441 pounds of potassium permanganate were applied to the trenches and boreholes. November 2015 to The SSD system equipment is delivered to the site, the system equipment pad is constructed, City of Raleigh permits are January 2016 obtained, electrical modifications are completed, and the SSD system piping is connected to the SSD equipment cabinet. February 3, 2016 The SSD system is started up. The SSD system accumulates water after a significant rain event on February 4, 2016. The flow is reduced to minimize February 4-29, 2016 water accumulation. URS oversees the modification of the SSD system piping, which included a 2-inch diameter slotted screen piping installed within the 4-inch diameter system piping to reduce the amount of sediment/fines entering the SSD system piping. All wells are redeveloped. March 3, 2016 URS oversees the completion of a comprehensive groundwater gauging event, a limited groundwater sampling event of select site wells, and waste pickup. URS also completes operation and maintenance (O&M) on the SSD system. March 8, 2016 URS performs additional O&M on the SSD system. URS attempts to collect three indoor air samples from the former dry cleaner and two adjacent tenant spaces (Shear Shack March 17, 2016 and Beauty Supply); however, an open door is observed halfway through indoor air sampling and recollection is discussed with DSCA. August 1, 2017 AECOM collects one 8-hour indoor air sample from the Shear Shack. The SSD system was operational at this time. November 28, 2017 AECOM collects three 8-hour indoor air samples from the following tenant spaces: the former dry cleaner (vacant), Shear Shack, and the former Beauty Mega Store (vacant). The SSD system was operational at this time. July 2017 - February Routine O&M performed on the SSD system. 2019 AECOM collects one 8-hour indoor air sample from the former drycleaner (currently Craft Habit) and collects one effluent April 3, 2019 sample (EFF-1). After sample collection, the SSD system was turned off and two Austin AirSystems HealthMate portable air filtrations units deployed in the Craft Habit tenants ace. April 24, 2019 After two weeks of air filtration unit operation, AECOM collects one 8-hour indoor air sample from the former drycleaner (currently Craft Habit) to confirm effficacy of the portable air filtration units. June 17, 2019 AECOM collects seven subslab vapor samples (SSV-1 to SSV-5 and SSV-12 to SSV-13) from various tenant spaces within the Gateway Plaza. SSV-9 not collected due to a summa canister malfunction. AECOM collects one indoor air sample from the former Shear Shack/Institute of Divine Metaphysical Research (currently June 26, 2019 Mordecai Beverage Company). AECOM deploys Hydrasleeves in all site monitoring wells, except for MW-6, MW-7, MW-15, MW-16, and DMW-18, which could not be located due to parking lot construction activities. June 28, 2019 AECOM completes a comprehensive groundwater gauging and sampling event for iron and manganese via Hydrasleeves deployed on June 26, 2019. AECOM collects five subslab vapor samples (SSV-6 to SSV-8 and SSV-10 to SSV-11) from various tenant spaces within July 1, 2019 the Gateway Plaza. Well modifications (i.e., well pad repair) completed by a NC -licensed driller at MW-1R, MW-5, MW- 12, MW-13, and DMW-18 due to parking lot construction activities. Monitoring wells MW-6, MW-7, MW-15, and MW- 16 could not be located for proper abandonment.
