OBSERVED VIOLATIONS:
.0835(b) All permitted septage management firms shall display lettering on each side of every pumper vehicle operated
by the firm. The lettering shall include the firm name, town name, phone number and septage management firm permit
number as shown on the firm application. All lettering required by this Rule on the pumper vehicle shall be no less than
three inches in height and legible, distinguishable from the background, and not obstructed from view.
Inspected the new vehicle tag#YA117467 – on both sides of this vehicle the town name (Archdale) contained some letters
that were less than three inches in height. Correct the town name on both sides of this vehicle and submit a photo of
the corrected lettering to this office by January 30th. Gave Mr. Queen my contact info.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected the new vehicle, tag#YA117467, VIN#1FDXF7DX3MDF07109, 2500 gallon capacity. The tank
appeared structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. Lettering
issue as noted on the first page of this report.
2. Inspected the older pump truck, tag#YA136399, VIN#1FVACXDC46HV75659, 2600 gallon capacity – info
correct as per the firm application. The tank appeared structurally sound & securely attached. Outlet covers
were on and no drips/leaks were noted. The lettering is correct on both sides of this vehicle per info on the firm
application.
3. Reviewed the truck logs at this time. Discussed the required information to be maintained for all septage pump
jobs. Missing items included the amount of gallons pumped at each job and where septage was disposed
of (which WWTP) for each job. Gave Mr. Queen an example truck log for his use.
4. Discussed the permitting & management of septage land application sites and also the Rule regarding loaded or
partially loaded trucks can be located on property owned by the permit holder for up to 7 days.
5. Checked with the Raleigh Central office in regards to the payment of fees for two vehicles. It appears that the
fee was paid for two vehicles for 2020. However it appears at this time that no fee or renewal application has
been received for 2021. Please contact the Raleigh Central office asap to resolve this situation.
