PURPOSE OF SITE VISIT:
Partial Facility Compliance Inspection.

STATUS OF PAST NOTED VIOLATIONS:
None

OBSERVED VIOLATIONS:
None

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:

01 - This facility has a locking Gate at roadside entrance for security.
02 - Signage has required information.
03 - Facility is permitted to receive waste from Perquimans, Chowan and Gates Counties.
04 - Waste must be transported to The East Carolina Regional Landfill (0803-MSWLF-1993) in Bertie County.
05 - Windblown trash was not an issue.
06 - No leaks noted in loaded trailers.
07 - The tipping floor and loading bay were clean.
08 - Screening records were not reviewed during this partial facility compliance inspection.
09 - Electronics are being collected at this site. Managed well.
10 - Scrap metal area is being managed well. All white goods containing CFC’s were upright.
11 - Yard waste is being managed well. Has fire lanes, and temperature records are being recorded but not reviewed
during this partial facility compliance inspection.
12 - Facility has 2 Certified Transfer Station Operations Specialist: Charlie Rome: expires 3/12/2022, David Lowe:
expires 5/25/2022.
13 - Interior walls of Transfer Station have not been washed since the last inspection.
14 - Scrap tires area is being managed well.
15 - CFC removal records were not reviewed during this partial facility inspection.
16 - PTO and approved plans onsite but not reviewed during this partial facility inspection.
17 - Load in and out records available but not reviewed during this partial facility inspection.
18 - Land Clearing waste area looks good.
19 - Concrete area looks good.
20 - Overall management of this facility is good.
21 - Covid-19 Recommendations were followed during this inspection.
