with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected a new vehicle to be added to the firm permit. Tag#FA1375, VIN#3C7WRMCLXKG646712, 850
gallons. The tank is structurally sound & securely attached. Outlet covers were on and no drips/leaks were
noted. The lettering is correct on both sides of this vehicle per the info on the firm application. This vehicle is in
compliance at this time.
2. Did not inspect the other permitted vehicles or review truck logs at this time.
3. The firm permit renewal is underway at this time. Ensure that the correct tag & VIN are listed for all the
vehicles to be used in 2021. Ensure that the Raleigh Central office receives an authorization for the Enviro
Choice facility to be added as a disposal location in 2021.
