STATUS OF PERMIT:
The Solid Waste Section sent a letter to Mr. Darrell Cash on July 24, 2000, stating that they had received the Notification
signed by Mr. Cash. The Notification was recorded in Deed Book 1177 and Page 0679 at the Register of Deeds in Louisburg,
NC.
PURPOSE OF SITE VISIT:
Provide technical advice and investigate a fire that was reported to the Section on November 30, 2020.
STATUS OF PAST NOTICE OF VIOLATIONS

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
On November 30, 2020, Mr. Heath Cash contacted Ms. Thompson to report a fire that had started at the site. Mr. Heath
Cash stated that the fire had started sometime during the night of November 25, 2020. Mr. Cash stated that the cause of
the fire was likely spontaneous combustion; no one was smoking near the landfill nor was it caused by a lightning strike.
According to the Fire Incident Report, approximately 75% of the landfill had burned. As soon as the fire was discovered,
Mr. Cash contacted the local Fire Department, who in turn contacted the Forestry Service. The Fire Department applied
water to the site, while the Forestry Service wet and monitored the adjacent pasture lands and forested areas. NCDOT
was also contacted and put up "Caution Dense Smoke" signs along M.C. Wilder Road. The Fire Department also contacted
the nearby airport to warn them of the smoke. A pump was placed in an adjacent irrigation pond to supply firefighting
hoses and sprinklers. Approximately 800,000 gallons of water were applied to the site prior to the Section's arrival. Based
on the advice of the Fire Department, Forestry Service, and Stacey Smith of Smith + Gardner, Mr. Cash excavated areas
of waste, flooded the excavated hole with water then covered with soil.
1. Upon arrival at the site, smoke could be seen rising from the northern, southern, and western side slopes. According
to Mr. Cash, virgin soil is located in the northeastern portion of the landfill.
2. The areas of stockpiled waste that were located on top of the landfill in the October 15, 2020 inspection had been
removed for safety during the fighting of the fire. Ms. Thompson and Mr. Hammonds walked around the entire facility
to assess the fire damage.
3. The near vertical western side slope, identified in the October 15, 2020 inspection, had been burned. Mr. Cash stated
that he had excavated some of the waste that had not burned then flooded the area and covered with soil.
4. Several larger stumps that had not been charred were located along the upper most point of the southern side slope.
Mr. Cash asked if the uncharred waste could be removed for grinding. Mr. Hammonds and Ms. Thompson agreed
that the uncharred waste could be removed and ground. This would help reduce the fuel for the fire.
5. Mr. Cash then showed Mr. Hammonds and Ms. Thompson the starting point of the fire along the southern side slope
of the landfill. Mr. Cash had excavated a trench near the starting point of the fire to remove the fuel and flood the area
with water. During our discussion near the starting point, an area of flames could be seen. Mr. Cash stated that he
would be applying additional water and mixing the dirt and waste to smother the fire, after our inspection.
6. Prior to ending the meeting, Mr. Hammonds stressed the importance of safety while fighting the landfill fire. Mr.
Hammonds stated that cover would be the best option to help smother the fire. The mixing of the waste with water
prior to the addition of cover would also help. Mr. Hammonds stated that we would return to the Site before the
holidays to re -inspect the site.
7. Mr. Cash stated they had not covered the waste in some time. Section staff explained the importance of proper
management and cover of the NLCID as well as operating in accordance to the rules.
8. Efforts to extinguish the fire should continue keeping the safety of site staff in mind.
