STATUS OF PERMIT: Active: Issued November 8, 2017
The Permit to Operate shall expire December 10, 2022. No later than June 13, 2022, the permittee must submit a request to the Section for permit review and must update pertinent facility plans including, but not limited to, the facility operation plan, contingency plan, and/or closure plan.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. David Powell visited the Cumberland County HHW facility for an inspection. Facility has a gate and is fenced.
2. The facility is open second Saturday, 8am to 12pm, and fourth Wednesday, 12pm to 4pm, each month.

3. EcoFlow is till used to remove the waste. NCDA collects the pesticides. Items in storage awaiting disposal all were labeled and had date. Good job.
4. Waste collected is removed about once a quarter. Good job because the permit limits waste being stored onsite to 180 days at most. Facility collects pesticides, corrosives, oxidizers, antifreeze, motor oil and flammables.
5. Inspection logs were reviewed after inspection at facility. Be sure this is done at least twice a month as required by permit (Permit Reference Part VII. 8). A copy of the permit was available onsite and a copy of the 40 Hr.
OSHA HAZWOPER Certification training for Neal Cunnington and Virgilio De la Serna as well. Eye wash station was checked and operated. Fire extinguisher is present and a 2020 label.

Inventory sheet for when waste is brought to site.
6. Inspector inquired about any spills, fires or vandalism in recent history at the facility. Staff indicated there was none. Good job.
7. Facility looked neat and orderly upon inspection. Good job.
