STATUS OF PAST NOTED VIOLATIONS:
1. RESOLVED: 15A NCAC 02C .0108(k) states: "All non -water supply wells, including temporary wells, shall be
secured with a locking well cap to ensure against unauthorized access and use." In a letter from Smith Gardner,
Inc., dated August 30, 2018, Joan Smyth certified that all landfill gas and groundwater monitoring wells at
the Holly Grove Landfill had been upgraded and/or repaired as necessary to ensure that each had been
secured with a locking well cap.
2. RESOLVED: 15A NCAC 02C .0108(o) states: "Each non -water supply well shall have permanently affixed an
identification plate. The identification plate shall be constructed of a durable, waterproof, rustproof metal or other
material approved by the Department as equivalent and shall contain the following information:
(1) well contractor name and certification number;
(2) date well completed;
(3) total depth of well;
(4) a warning that the well is not for water supply and that the groundwater may contain hazardous materials;

(5) depth(s) to the top(s) and bottom(s) of the screen(s); and,
(6) the well identification number or name assigned by the well owner."
In a letter from Smith Gardner, Inc., dated August 30, 2018, Joan Smyth certified that all landfill gas and
groundwater monitoring wells at the Holly Grove Landfill had been upgraded and/or repaired as
necessary to ensure that the required permanent identification plate has been affixed to each.
OBSERVED VIOLATIONS:
None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
1. The facility is a closed, unlined MSW landfill that ceased accepting waste in October 1993.
2. Photos included in this report were taken by Susan Heim on November 16, 2020 unless otherwise noted.
3. Groundwater monitoring is conducted semiannually, as required. The four most recent sampling events took
place October 15 — 18, 2018, March 25 — 27, 2019, September 16 — 20, 2019 and March 9 — 12, 2020. Reports
covering these monitoring events were reviewed online and verified.
Field Inspection:
4. The facility is protected by a series of locking gates and stretches of fencing to prevent unauthorized entry. Mr.
Brushwood stated that additional fencing and other security measures had been installed when facility staff
found evidence that 4-wheelers had been intruding into several areas of the site.
5. The landfill was found to be well maintained. No evidence of erosion was observed during this inspection.
6. Vegetative cover appeared to be well -established, and no areas lacking vegetation were noted during this
inspection.
7. Small trees and woody growth were observed in several areas along the toe of the landfill slope in the vicinity
of the edge of waste, and also in a vertical line, presumably beside a slope drain. Please ensure that all trees
and woody vegetation are removed from the landfill, and that the cap is repaired and restored after
removal is complete.
8. Several groundwater monitoring wells were observed during the inspection and were found to be locked and
properly tagged. In addition, each of these wells was easily accessible and free from intrusive vegetation.
