DATE AND TIME OF FIRE: 11/18/2020 @ 3:30 pm
HOW WAS THE FIRE REPORTED AND BY WHOM: The Equipment Operator notified our Operations Supervisor m person t a ere was a ,re rnsr e e raI er
that was being loaded.
LIST ACTIONS TAKEN: The Equipment perator pulled the trailer away rom any stru
the trailer. The Operations Supervisor called the Fire Department to report the fire.
WHAT WAS THE CAUSE OF THE FIRE: I The cause of the fire could not be detennined.
DESCRIBE AREA. TYPE AND AMOUNT OF WASTE INVOLVED:
The fire was contained inside of the loaded trailer. The trailer was loaded with MSW.
WHAT COULD HA VE BEEN DONE TO PREVENT THIS FIRE:
We did not notice any "hot materials" while trucks were unloading or while loading the trailer. I am not sure
that anything could have been done to prevent this fire.
DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS:
We will continue to screen loads and be very observant to waste that is being brought into the facility.
