1. Introduction
O'Brien & Gere Engineers, Inc. (O'Brien & Gere) was retained by Northern
Telecom/Nortel (Nortel) in December 1997 to perform an asbestos survey
of the Nortel facility located at 4001 East Chapel Hill -Nelson Highway in
Research Triangle Park (RTP), North Carolina_ It was O'Brien & Gere's
understanding that the Nortel RTP facility was to be scheduled for a major
renovation during 1998 and that this asbestos survey was necessary to
classify potential asbestos -containing materials (ACM) prior to the
renovation activities. As such, the asbestos survey was conducted in
compliance with the Occupational Safety and Health Act (OSHA) standards
contained in 29 CFR 1910.1200 and the Asbestos Hazardous Emergency
Response Act (AHERA) requirements.
For clarity, this report has been subdivided into the following sections:
• Section 2 - Building description, which provides general information
concerning the size, age, and construction materials for the building.
• Section 3 - Asbestos -containing material survey, which discusses the
suspect materials that were sampled, the suspect materials there were not
sampled, and the material which were confirmed to be ACM or non -
ACM based on the January 1998 analytical results.
• Section 4 - Survey conclusions and recommendations, which provides a
listing of the known ACM and the suspected ACM (which was not
sampled based on the January 1998 asbestos survey) and which provides
recommendations for addressing these materials prior to the renovation
activities.
• Section 5 -Subsequent asbestos abatement activities, which describes the
asbestos abatement activities which have been conducted subsequent to
the January 1998 asbestos survey.

2. Building description
2.1. Building composition

O'Brien & Gere performed a preliminary site wallcthrough of the Nortel RTP
facility and its buildings on December 2, 1997. Based on the preliminary
survey, one large manufacturing/office building and three small guard
houses were identified at the facility. The layouts for the large
manufacturing/office building are provided in Figures I through 7. (The
three small guard houses were included within the surveyed area, but are not
depicted on the figures.)
The large manufacturing/office building was identified to comprise a total
of 776,000 ft' of enclosed space, of which: 4I4,000 ft' is designated for
office uses; 337,000 ft- is designated for manufacturing purposes; and
15,000 ff- is designated for laboratory uses. The following table provides a
description of the different sections of the large manufacturing/office
building, which were all constructed during the 1980 to 1994 time period:

3. Asbestos -containing material survey
After the preliminary site walkthrough on December 2, 1997, O'Brien &
Gere followed -up at the Nortel RTP facility with an ACM survey on January
5 and 6, 1998. During the two-day survey, a total of 160 samples of potential
ACM were collected and shipped off -site to Carolina Environmental, Inc. of
Raleigh, North Carolina for analysis by polarized light microscopy (PLM)
for the presence of asbestos. (Carolina Environmental, Inc. subdivided
several of the samples to better characterize the layers of potential ACM. As
such, a total of 232 samples were analyzed.)
3.1. Suspect materials included in sampling
The following materials were identified as suspect during the preliminary
building survey performed on December 2, 1998 and were sampled during
the subsequent ACM survey conducted on January 5 and 6, 1998:

Suspect ACMs were divided into homogeneous sampling areas.
Homogeneous sampling areas are those areas that contain materials which
are uniform in function, age, and physical appearance. Homogeneous areas
may be adjacent to each other or in separate areas within a building. A total
of 160 samples were collected from the homogeneous sampling areas shown
on Figures 1 through 7 and 232 samples were analyzed (due to material
layering within the collected samples).
3.2. Suspect materials not included in sampling
During the preliminary survey and the subsequent ACM survey, the
following suspect ACM was also identified:
• Built-up roofing materials
• Sealants for roof equipment
• Insulation for fire doors
• Sprayed -on insulation on the structural members above the second floor
ceiling tiles and overlying sheetrock
• Transite in laboratory hoods
• Transite in shock -resistance testing equipment.
Sampling of these materials would have required destructive testing during
the ACM survey. Instead, it is recommended that these materials be duly
noted and that ACM sampling and analysis be performed immediately
(within the week) prior to the proposed renovations (to identify ACM
content) which would affect these areas.
3.3. Confirmed asbestos -containing materials
Based on the analytical results (which are provided in Attachment 1), the
following materials were confirmed to be ACM by PLM analysis based on
sampling conducted on January 5 and 6, 1999:

The positive ACM areas from the January 5 and 6, 1998 survey are denoted
by red sample designations and red shading on Figures 1 through 7.
It is recommended that the above -referenced materials be removed prior to
the proposed renovation materials. O'Brien & Gere can assist with the
identification and removal of these materials, if needed.
4.2. Continued suspect asbestos -containing materials
During the preliminary survey and the subsequent ACM survey, the
following suspect ACM was also identified:
• Built-up roofing materials
• Sealants for roof equipment
• Insulation for fire doors
• Sprayed -on insulation on the structural members above the second floor
ceiling tiles and overlying sheetrock
• Transite in laboratory hoods
• Transite in shock -resistance testing equipment.
Sampling of these materials would have required destructive testing during
the ACM survey. Instead, it is recommended that these materials be duly
noted and that ACM sampling and analysis be performed immediately
(within the week) prior to the proposed renovations (to identify ACM
content) which would affect these areas.

4. Survey conclusions and recommendations
The office areas were inspected for floor tile through a random inspection
process, which required the removal of the overlying carpeting. Based on
this inspection procedure, the areas denoted in Figures 3 through 6 were
found to contain asbestos -containing floor tile and mastic. However, it
should be noted that a complete inspection is recommended as the
carpeting is being removed prior to the renovation work.
In addition, as noted on Figure 5, one area of floor the and mastic on the
first floor in the West Office Building was not specifically sampled due to
its resemblance to floor the identified and sampled in the basement. The
floor the and mastic in the basement was identified to be ACM. It is
recommended that the floor tile and mastic on the first floor be sampled
to confirm whether it is ACM prior to the renovation activities. This
additional sampling may confirm the floor tile and mastic to be non -ACM,
which would reduce the removal and disposal costs.
4.4. Inspection accreditation
This inspection was conducted by Brantley D. Chesson, who holds
Accreditation Number 10002 with the Health Hazard Control Branch of
the North Carolina Department of Environment and Natural Resources.

5. Subsequent asbestos abatement activities
The purpose of this section is to document the asbestos abatement activities
which have been conducted at the Nortel RTP facility since the January 5
and 6, 1998 asbestos survey. As such, this section describes the measures
taken to remove, encapsulate, and/or include identified ACM within an
operations and maintenance plan.
5.1. July 13 - 24,1998 abatement project
O'Brien & Gere and NEO Corporation were retained to remove ACM floor
tile and mastic from the first floor of the Nortel RTP facility's east office
building on July 13 through 24, 1998. This abatement project resulted in the
removal of ACM floor tile and mastic from the area designated with green
shading on Figure 3. Information concerning the removal procedures,
disposal documentation, and certifications ofthose personnel involved in the
ACM removal project are provided under separate cover.
5.2. September 4 - 6, 1998 abatement project
O'Brien & Gere and NEO Corporation were retained to remove ACM floor
the and mastic from the basement floor of the Nortel RTP facility's west
office building on September 4 through 6, 1998. This abatement project
resulted in the removal of ACM floor tile and mastic from the area
designated with purple shading on Figure 7. Information concerning the
removal procedures, disposal documentation, and certifications of those
personnel involved in the ACM removal project are provided under separate
cover.
