PURPOSE OF SITE VISIT: Routine/Follow up
STATUS OF PAST NOTED VIOLATIONS: A Notice of Continuing violation was issued on 1/2/2020 for violations of the following rules: 15A NCAC 13B
.0842(a)(7), 15A NCAC 13B .0842(a)(12) and 15A NCAC 13B .0842(a)(15). Work has been done to resolve these violations. No new violations have been observed in this area.
OBSERVED VIOLATIONS:
Violations of the following rule were observed during the site inspection on 3/19/2020 for failure to maintain the crop described by the Nutrient Management Plan.
15A NCAC 13B .0842(a)(17) approved nutrient management plans shall be followed;

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) An appropriate Septage Land Application Sign was present at the entrance to the site.
2) Roads were passable.
3) Some field markers were run over or missing. Any field markers knocked down or out of place need
to be restored.
4) No discharges were observed outside of the permitted field boundaries.
5) Applications appeared to be following the permit and nutrient management plan within the
permitted boundaries.
6) The crop is not being maintained as described by the nutrient management plan. No evidence of
winter crop overseeding was observed on any permitted field. The vegetation on Fields 1, 2 and 3
was dominated by weeds. All fields require weed control. The unpermitted field to the west of field
1 was seeded as was required by the Notice of Continuing Violation issued 1/2/2020. The buffer area
adjacent to the western edge of field 1 was recently plowed where the soil surface was left bare.
7) pH not checked. No application at time of inspection.
8) No objectionable odors noticed.
9) Setbacks not checked.
10) Logs were not checked
