PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: None. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Page 1 of 2 FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management NORTH CAROLINA Solid Waste Section ADDITIONAL COMMENTS 1. All photos included in this report were taken by Susan Heim during the facility inspection on 10/30/2020. 2. The facility is permitted to receive, consolidate and transfer yard trash (as defined in 15A NCAC 13B .0101(55)) for storage and removal. The site receives and manages leaves collected curbside by the City of Winston-Salem Sanitation Division. 3. Operations at the site are managed by Wallace Farms, Inc., a contractor that also manages the city's Overdale Road Yard Waste Facility (permit # 34-09) and Forum 52 Large Type 1 Compost Facility (permit # 34-23). 4. The site is secured by a chain link fence that encloses the property, and unauthorized access is prevented by two locking gates that are kept locked except when facility staff or Wallace Farms staff are present. 5. The site was observed to be clean and well -maintained. 6. City trucks unload leaves at the site and push them into piles. Wallace Farms staff then manage the leaf stockpiles during their temporary storage at the site, and load them into trucks, transporting them as needed. 7. The facility had begun receiving a few loads of leaves during the week of this inspection. Several small truck loads had been dumped in separate piles by city trucks. Mr. Cain stated that staff from Wallace Farms were due at the site within hours to set up the ramp and consolidate the small piles for ease of loading. 8. A white dashed line marks the edge of the required buffer at the site, and all waste was well within the permitted operations area. In addition, a truck lane had been established using road cones so that trucks are directed to the desired location for unloading and exiting the site. 9. The entire site is paved with asphalt, with four small concrete pads surrounding the operations area. 10. Coarse gravel had been spread to fill low areas where the old asphalt had eroded. During previous inspections, this had been observed to be an effective means of preventing water from ponding in and around the leaf piles. At the time of this inspection, some ruts and potholes were visible on the west side of the operations area. Mr. Cain pointed out the gravel pile staged at the western end of the property that would be used to fill these remaining spots. Please ensure that the potholes and low spots in the operations area are filled with gravel and leveled to prevent water from ponding in and around the leaf piles. 11. The site is sloped so that surface water is directed to the city's storm drains. 12. The facility annual report for FY 2019-20 was reviewed. The report indicated that 16,251.60 tons of leaves were received at the site during the last operating season.
