The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The field has been harvested but the rye has not yet been planted. An adequate cover crop shall be established
for land application to occur this winter. Noted scattered thin patches of Bermuda and areas of weeds & other
vegetation. Contact Georgia Love (regional agronomist) after this winter for advice on weed control and
guidance on measures to improve the Bermuda cropstand in the damaged areas.
2. There are boundary markers that have vegetation growing up around the posts – remove the excess vegetation.
3. No sign of recent or heavy application. No sign of land application outside the permitted boundaries. It appears
that the approved setbacks are being met – the setbacks were not measured at this time. No odors noted.
4. The required signs are posted and the access road is adequate.
5. Discussed that soil samples shall be taken this fall by the operator. Also discussed that an authorization
form for 2021 shall be submitted if any firm other than Cooper Septic Tank Service shall be utilizing this
site. At this time additional site logs shall be submitted by Emily Cooper (discussed which months are
required). Ms. Cooper stated that she had attended the training class in Shallotte.
