requirements is hereby registered as a Portable Sanitation Firm
in the State of North Carolina.
This permit to operate a Septage Management Firm is issued to the above named person, business or entity alone and is not transferable to any other person, business or entity.
Firm operation shall be in accordance with the provisions of N.C. General Statute 130A-291.1 - 130A-291.3, Title 15A of the N.C. Administrative Code 13B .0800 et.seq.,
conditions of the permit, and representations made in the application and accompanying documents for a permit.
The permit holder is authorized to discharge septage only at the locations(s) listed below:
1.Long Creek Wastewater Treatment Plant, Dallas NC
2.City of Monroe WWTP
This permit does not entitle the permit holder to operate a Septage Land Application Site, a Septage Detention or Treatment Facility, or any other solid waste management facility not specified herein.
Failure to operate as permitted may result in the Department suspending or revoking this permit, initiating action to enjoin the unpermitted operation, imposing administrative penalties, or invoking any other remedy as provided in Chapter 130A, Article 1, part 2 of the North Carolina General Statutes. This permit and registration expires on December 31, 2020.

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1.Looked at the following vehicles.

Tag# JC1315, VIN# correct per the firm application, capacity 1500 gallons (was incorrectly listed as 1100 gal)

Also looked at a vehicle listed under permit number# NCS-01398/Gastonia location

All of these vehicles were lettered correctly on both sides of the vehicles. Outlet covers were on and no
drips/leaks were noted. The tanks appeared structurally sound & securely attached. These vehicles are in
compliance at this time.
2.The following vehicles on the submitted list/application were not available for inspection.
Tag#JC1317, Tag#JH4426, Tag#JC2937 were informed that these trucks are located in Gastonia and should be
under permit number# NCS-01398.
Tag#JC1311 was informed that this truck is now located in Virginia.
Tag#JC2936, was informed that this truck is under the Monroe permit however it was in Gastonia on this day
and not available for inspection at this time – will need to be inspected in the future.
Ensure that the vehicle lists submitted with the applications for the permit renewals of both the Monroe
and Gastonia locations list the correct vehicles for each location.
3.Asked to review the truck pumping log and was shown the WWTP dump tickets which do not record all of the
required information. I gave Mr. Boyd an example truck log and discussed the information to be maintained
daily for each porta john route for each truck. Ensure that the required information for the truck pumping
log is maintained.
4.Sent Mr. Boyd an email concerning information on the remaining class locations for the continuing
education hours. Ensure that the required 4 hours of continuing education are obtained by the end of
2020.
