PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The facility is fenced and is secured by a locking gate when it is not open for operations. 2. The correct signage is located at the entrance of the facility. 3. Operating hours are 8:00am to 3:30pm, Monday through Friday. 4. The Permit to Operate was issued December 19, 2016 and shall expire on June 30, 2021. Mr. Frye was reminded that he needed to submit the permit renewal paperwork by the end of December 2020. 5. The permit, operations plan, and records were on site and were reviewed with the operator. 6. The facility is permitted to store up to 235,000 tires on -site at any one time based on a demonstrated financial responsibility of $352,000. 7. A minimum of seventy-five percent (75) of the tire -derived products must be disposed of in the Central Carolina Tire Monofill (Permit No. 43 04-TIRELF- 1992) or shipped off -site on an annual basis. Adjustments to this percent to allow additional amounts of storage or adjustments to allow additional length of storage time must be submitted to the section for review and approval. 8. The facility had multiple fire extinguishers at the facility. All appeared to be properly charged and maintained. The Spout Springs Fire Station provides fire protection services to the facility. 9. Incoming tires are segregated into three primary types: 1) tires suitable for resale or recapping, 2) passenger tires, and 3) tires from off -road equipment. 10. Passenger vehicles tires that are not culled for resale are shredded into drain field material or tire derived fuel. Tires too large for processing are split for disposal in the tire monofill. 11. No unacceptable waste was observed. 12. No vectors were observed during the inspection. 13. Tires were observed to be stored within an area of standing water, near the shredder. The area should be graded to promote drainage of surface water away from the tire storage area. 14. Digital photographs were taken during the inspection.
