PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None Pagel of 3 NORTH CAR()LINAD_E Q�� OepaNnent W Envimnmenbl Wel� OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. The facility is fenced and is secured by a locking gate when it is not open for operations. 2. The correct signage is located at the entrance of the facility. 3. Operating hours are 8:00am to 3:30pm, Monday through Friday. 4. The permit, operations plan, and records were on site and were reviewed with the operator. 5. Certified Landfill Operators: Brian Thomas, Certified Landfill Operations Specialist, LF-2011068, expires 10/21/2023 Dean Brewer, Certified Landfill Operations Specialist, LF-2017035, expires 04/11/2023 6. The facility maintains semiannual groundwater and surface water monitoring records. Records for the April 8, 2020 sampling event were verified. 7. One monitoring well, MW-16R, was not locked at the time of this inspection. Mr. Frye stated that the consultant had finished the groundwater sampling event the week prior. Mr. Frye also stated that he would have the consultant return to the facility to inspect the monitoring wells to make sure they were labeled and locked. On October 20, 2020, Mr. Brian Thomas provided a photograph of the new lock for MW-16R. 8. Permanent edge of waste markers are in place and maintained. 9. Side slopes are being well maintained and mowed. Areas of erosion are being repaired as they are observed. 10. The access roads to the monofill are being well maintained. 11. Employees working in the landfill receive necessary training and ongoing monthly training in landfill operations. 12. Digital photographs were taken during the inspection.
