The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Once a year on site meeting. Reviewed the land application site logs (combo log with pump job info) – it
appears that the required info is being maintained. Discussed that soil samples shall be taken by the end of
this year and that the required training hours shall be obtained.
2. The required signs are posted and the access road is adequate.
3. The left field has been cut but the hay has not yet been removed. The right field has not been harvested yet.
Discussed that the winter cover crop shall be established per the approved nutrient management plan.
Land application has occurred on the right field so it cannot be harvested for 30 days after the last discharge
event.
4. The Bermuda cropstand appears to have improved this year.
5. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are
being met but were not measured at this time.
6. The back corner post and the front corner post on the right side of the right field need to have vegetation
removed from around the posts. Excess vegetation has been removed from other field markers since the last
inspection.
