Application for permit renewal has been submitted to SWS and permit is currently under review.

OBSERVED VIOLATIONS:
1. Per 15A NCAC 13B .0105 (c) Vehicles or containers used for the collection of solid waste, and
transportation by whatever means, including highway, rail, and navigable waterway, shall be
constructed, operated, and maintained to be leak resistant in order to prevent the creation of a nuisance
to public health from the escape of solid, semi-solid, or liquid waste. In order to meet the requirement to
be leak resistant, the owner and/or operator of the vehicle or container shall adhere to the following
standards:
(3) The vehicle or container shall be equipped with seals, gaskets, or other devices pursuant to
manufacturer specifications in order to prevent the escape of liquids. Such seals, gaskets, and other devices
shall be maintained and replaced pursuant to manufacturer specifications.
(4) The vehicle or container body, waste holding area, and hopper, if so equipped, shall be free of holes,
cracks, rusting, corrosion, or other evidence of damage or weakness that may allow the escape of solid,
semi-solid, or liquid waste.
(6) The vehicle or container shall be loaded, transported, operated, and maintained to prevent the escape of
solid, semi-solid, or liquid waste to the environment.
(7) The vehicle or container shall be serviced, repaired, and cleaned to maintain sanitary conditions, to
preserve the integrity of the door seal, to prevent the accumulation of mechanical fluids, dirt, and filth on
the vehicle's exterior, and to prevent contamination of the environment by fluids.
Leaking was observed from transfer trailer with tag #83479-T (truck #15) during inspection. This trailer
cannot be put back into service until it is repaired.
Duplin County placed an order for two Mac Trailers in September and will rotate phase out older trailers when
received in approximately 45 days.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The facility is permitted to receive waste generated Duplin County.
2. The facility has proper signage.
3. Personnel are onsite during operating hours.
4. Records were not reviewed at this inspection.
5. The back wall of the transfer station above the trailer loading area have been damaged and need to be repaired. The
WTS is to be maintained in a clean and sanitary condition.
6. The windblown litter is being managed well. The perimeter of the facility looked good.
7. The waste type being accepted into the facility was MSW and C&D.
8. Pallets observed in MSW waste during inspection. Please ensure all staff are trained to identify unacceptable wastes
and are following facility operations plans in regards to the handling of the unacceptable waste materials.
9. The leachate collection system was reviewed and is operating properly at this time.
10. There are two fire extinguishers available for fire control. A fire house is also available for fire control.
11. The access around the facility is stable and with minimal soil erosion.
12. Digital pictures were taken during the inspection.
