The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1- Once a year on site meeting. Reviewed land application site logs at this time. It appeared that some of the
discharge events since 2019 have been recorded on an incorrect form. Please redo these specific sheets so that
complete & accurate site logs will be available for the site permit renewal process next spring.
2- The Bermuda stand looks adequate on the three fields. The winter cover crop needs to be planted by November
per the approved nutrient management plan.
3- Noted a missing field post in front of the storage tanks (cut post on ground) on Field A. Noted a missing
corner post and also a post leaning on the ground on Field C (on the woodline side going towards the gap
with Field B). Also noted a couple posts at the back of Field B that were obstructed with excess
vegetation. Replace/repair the field posts as needed. It appears that the approved setbacks are being met but
were not measured at this time.
4- No sign of land application outside the permitted boundaries. No sign of recent or heavy application. No odors
noted.
5- The required signs are posted and the access road was adequate.
6- Hay bales were noted on site but was not hay harvested from the septage fields.
7- Ms. Locklear stated that she had attended the January training class in Hickory.
8- Soil samples shall be taken before the end of this year.
