Facility, Permit No. 1205T-TRANSFER-1998, for failure to submit their Facility Annual Report by
August 15t, as required each year by North Carolina General Statute130A-309.09D(b).
This letter serves to confirm that the requirement in the Notice of Deficiency dated September
