Active, Life of Site Permit issued March 27, 2019 and expires August 28, 2056. This permit is in the names of: Currituck County (owner) and Republic Services of North Carolina, LLC (operator).
PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection.
STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS:
Permit violation; part 6 – Items 6 - b, and 7 - a, c and e.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 - This facility compliance inspection was conducted with Leighton Earwood, Currituck Solid Waste Director. 02 - Minimal windblown trash was noted during this inspection around the transfer station main yard.
03 - There was a larger amount of windblown trash found at the trailer storage area that needs to be picked up daily (observed permit violation 7 e). 04 - Tipping floor and loading bay were clean as they can be during working hours. More attention needs to be done behind the push wall. 05 - There was old waste on the top of the recently turned I-Beam that needs to be cleaned daily (observed permit violation 7 a). 06 - One leak was detected on stored (loaded) trailer #171 with no containment trough or barrel placed under it to capture the leachate discharge (observed permit violation 7 c). There was one trough found that was full of leachate in the weeds that needs to be emptied into the leachate system. All trailers were tarped. 07 - Fire suppression equipment is available with several charged fire extinguishers and water hoses at the tipping floor and there is one charged fire extinguisher at the scrap metal area by 2 fuel tanks. 08 - Certified Transfer Station Operation Specialist: David Lowe, expires 5/25/2022, Charlie Rome, expires 3/12/2022 and Taylor Copeland, expires 11/14/2022 and Martyn Mitchell, expiration date not gotten today. 09 - Scrap metal pile has been reduced since the last inspection. Still need to clean up at the back of the pile as noted on the last inspection form. 10 - The white goods area was also inspected and found that all white goods containing CFC’s were upright until the CFC has been removed. These items are marked with a red X and a small label after the CFC is removed and then they
are place on the scrap metal pile for removal and sale. 11 - The yard waste volume has been reduced since the last inspection. 12 - There was quite a bit of non-conforming waste noted in the Yard Waste piles. It appears that most of this non-conforming waste is coming from the County Public Convenience Centers which are Contract Operated by Waste Management according to Leighton Earwood. Better screening at these site is needed to prevent a violation. Also, one load was screened during this inspection and found 12 bags of leafs in this load alone. This load came from Moyock, see pictures attached. 13 - Scrap tires are stored in enclosed box containers. 14 - Incoming load receipts were not reviewed during this inspection.
15 - Screening records were reviewed during this inspection. There were no screening records from August 19 to September 15, 2020. Operator stated they were out of forms and had ask for them (observed permit violation 6 a). 16 - Roads are constructed of all-weather materials and maintained well. 17 - Facility has adequate signage with required information. New directional signs have been installed.
18 - Scavenging of waste at the transfer station may be going on. Found two fishing rods placed on the wall during this inspection.
19 - Overall management of this facility had improved but I see a downward trend due to items listed on this inspection report. 20 - Digital pictures were taken during this inspection and some are attached to this report.

• Trailer #171 leaking without any leachate trough or other item to catch this leachate. Trailer need to be repaired. Leachate was contained to road area.
