OBSERVED VIOLATIONS:
1. .0835(b) All permitted septage management firms shall display lettering on each side of every pumper vehicle
operated by the firm. The lettering shall include the firm name, town name, phone number and septage
management firm permit number as shown on the firm application. All lettering required by this Rule on the
pumper vehicle shall be no less than three inches in height and legible.
The town name on vehicle tag#YA159728 is less than three inches in height on both sides of the truck. Correct
the town name on both sides of this vehicle to at least three inches in height. Submit a photo of the
corrected lettering to this office by September 30th.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected replacement vehicle tag#YA159728, 4000 gallon capacity, VIN#1NKDL40X9CJ333212.
The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted.
The lettering was correct except for the issue noted on the first page of this report.
2. The other vehicle was not inspected at this time and the truck logs were not reviewed – both were
inspected/reviewed in November 2019.
3. The required continuing education hours for the firm permit shall be obtained by the permit holder before the
end of 2020. Instructed Mr. Carter to contact the associations for current information regarding the remaining
class locations in 2020.
4. The truck inspected today is a replacement vehicle for vehicle tag#YA138366 which is currently out of service.
Discussed with Mr. Carter that the firm permit renewal & fee that is submitted this winter should accurately
reflect what vehicles may be used at any time in 2021 for septage pump jobs.
