1.0 OVERVIEW
This Construction Documentation Report has been prepared to document the construction of an
expansion of the landfill gas (LFG) collection and control system (GCCS) located on the Phase
4A municipal solid waste (MSW) landfill unit of the Johnston County Landfill facility near
Smithfield, North Carolina. The landfill facility is owned and operated by Johnston County under
State Solid Waste Permit No. 51-03. Frontier Operation Services (Frontier) owns and operates
the landfill gas management system under agreement with the County. Original approval for
construction of the GCCS was issued by the North Carolina Division of Waste Management
(NCDWM) on February 18, 2011 (see Appendix A).
2.0 PROJECT DESCRIPTION
2.1 General
The GCCS expansion (layout per DGP) includes the installation of following components:
• Seven (7) LFG extraction wells (EW-201 - EW-207)
• Approximately 2,050 LF of LFG transmission piping (HDPE (DR 17)):
o 8-inch diameter: 1,550 LF; and
o 4-inch diameter: 500 LF.
2.2 Reference Documents
The project was constructed in accordance with the following documents:

3.0 SUMMARY OF CONSTRUCTION ACTIVITIES
Major elements of the project are discussed below. Photos documenting the construction can
be found in Appendix B.

3.1 LFG Extraction Wells
Prior to drilling, the location and elevation for each LFG extraction well was surveyed by
Jerry Ball Land Surveying. The survey information was verified and approved by Tent
Engineering (also reviewed by S+G) to confirm the allowable well depth'.
A total of 7 LFG extraction wells having 36-inch diameter bore and a total length and
approximate total screened length of 485 feet and 308 feet, respectively, were installed
by Kellett's Well Boring, Inc. under contract to Advance One Development, LLC (Advance
One) between June 30th and July 7th, 2020. These installations were observed by S+G
(Britt Ransom) or S&ME (Ben Best). Spoils generated from drilling operations were
inspected by an accredited asbestos inspector in accordance with the Asbestos Work
Plan. No suspect asbestos containing materials were observed during this project. Well
logs are provided in Appendix C.
Once each well was drilled, Kellett's Well Boring and Advance One performed
completion activities which were generally as follows (given dimensions are
approximate):
• 1 foot of No. 3 (non -calcareous) stone was placed in the bottom of the well bore.
• The pipe casing (6-inch diameter SCH 80 PVC) was then installed with the
perforated section ending between 20-30 feet below the ground surface and solid
pipe extending vertically to the surface.
• The well was then backfilled with additional No. 3 stone to between 18-28 feet
below the ground surface (about 2 feet above the perforated section).
• 1 foot of soil backfill was added.
• A 2 foot thick expanding foam seal was added.
• Soil backfill was added to 5 feet below ground surface.
• A 2 foot thick expanding foam seal was added.
• Soil backfill was added to the ground surface, compacted, and sloped away from
the well casing.
• Following well completion, an approximately four (4) foot section of solid casing
remained above ground as a stickup.
• Each well was fitted with a vertical wellhead assembly and connected to
transmission piping.
' The total targeted well depth was determined by taking the difference of the existing ground elevation
and the top of protective cover (from prior record drawing information) and deducting a buffer of at least
twenty (20) feet.

3.2 LFG Transmission Pioin
Advance One installed the HDPE transmission piping (including 4-inch and 8-inch
diameter HDPE (DR 17) pipe) during July 2020 and made the corresponding connections
to newly installed LFG extraction wells and the pre-existing LFG piping at various
locations. At each of two connections with pre-existing header piping a butterfly valve
was installed for control of the system.
Upon completion of the transmission piping, Advance One performed a pressure test of
the piping, which was observed by S&ME. Documentation of the pressure test is
provided in Appendix D.
Record drawings, prepared by Tent Engineering, showing the location of LFG extraction
wells and transmission piping are provided in Appendix E.
4.0 DOCUMENTATION ACTIVITIES
In satisfying the requirements of the site's approved CQA Manual, the following activities were
performed:
Observation and documentation of the installation of LFG extraction wells, system
piping, and valves.
Review of record drawing.
Preparation of this documentation report.
5.o LANDFILL GAS (LFG) COLLECTION G CONTROL SYSTEM MATERIALS
S+G observed the LFG extraction well materials installed by Advance One. S+G observed the
LFG piping after installation had taken place. The selected materials included the following:

The Division of Waste Management (DWM), Solid Waste Section has completed reviews of the permit
modification application titled:
• Solid Waste Permit Modification, Landfill Gas Collection and Control System, Johnston
County MSW and C&D Landfill Facility. Dated November 30, 2010 and revised through
January 2011. (Doc ID 12972).
SCS Engineers, PC (SCS) in Charlotte, North Carolina on behalf of Johnston County prepared and
submitted the applications and requested approval for constructing and operating the landfill gas collection
and control system (LFGGCs) project at the above -referenced landfill facility.
Based on the reviews, the Solid Waste Section hereby grants Johnston County Landfill the authorization
for installing the proposed LFGCCs in accordance with the above -referenced document (Doc ID 12972).
An electronic pdf format copy of the approved document can be obtained on the DWM's website located at
hgp:Hacp.ncdenr.ora/pls/apex/f?p=2136:25:1590838597629807. Please note that this authorization for the
installation of LFGCCs does not remove any responsibilities Johnston County may have to comply with
any permit condition, or any local, state or federal regulation.
Upon completion of the construction of LFGCCs, the Solid Waste Section may grant Johnston County
Landfill a final authorization to operate the constructed LFGCC system. This operating authorization will
be issued pending receipt and approval of the following:
1. The Construction Documentation Report of LFGCCs project described in the approved application
document (Doc ID 12972) and as -built drawings of piping alignments and locations of wells,
valves, and sumps prepared by a surveyor licensed in the State of North Carolina.

2. The copy of air quality permit — Title V Permit approved by the North Carolina Division of Air
Quality.
3. The copy of the approval letter of the asbestos control plan for this LFGCCs project issued by the
Health Hazards Control Unit of the North Carolina Division of Public Health.
4. The copy of the letter approving the Erosion and Sediment Control Plan for this LFGCCs project
issued by the Land Quality Section, Division of Land Resources, if required.
The modified Operations Plan to the existing "Permit Amendment Application, Johnston County
MSW Landfill, Phase 4A, Cell 3 " approved by the Solid Waste Section in January 2009 by
describing how the operating LFGCCS project will be coordinated with the active landfill
operation at landfill facility, such as security/limit access to the flare station, training personnel
working around the wells and sumps, fire and emergency responses, and routine inspection and
maintenance.
6. The modified Closure and Post -Closure Plan to the existing "Permit Amendment Application,
Johnston County MSWLandfill, Phase 4A, Cell 3 " approved by the Solid Waste Section in January
2009 by including:
• Descriptions of steps necessary to decommission the components of the LFGCCs
(wells, piping, holding tanks, flare, blower, instrument panels, etc.) and restoration of
the final cover system if it is damaged by removing collection system at the end of their
useful life.
• The revised cost estimates and updated financial assurance mechanism including the
costs for decommissioning the components of the LFGCCs and repairing the final cover
system at the end of their useful life.

dated May 18, 2020. Based upon the information that was submitted I have a couple of comments: 1.)
Change OEE/EH Services Branch to EHS/Health Hazards Control Unit. 2.) Change the telephone number
from 919-707-5950 to my direct line 919-707-5972. 3.) If you discover asbestos at an active drill site
consider abandoning the site unless you need to drill in that exact location. 4.) Follow the OSHA signage
requirements for asbestos because there is no signage using the word "Potential".
Please adhere to the.plan as submitted to our office and keep the plan at the job site. Should there
be modifications to the plan that will impact asbestos, please inform us of the changes, in writing, before
implementing such changes.

GENERAL
1. ENTIRE INSTALLATION, INCLUDING MATERIALS, EQUIPMENT AND WORKMANSHIP, SHALL CONFORM WITH
THE MOST RECENTLY ADOPTED SPECIFICATION OF CODE AND WITH ALL APPLICABLE LAWS, CODES, AND
REGULATIONS AND REGULATORY BODIES HAVING JURISDICTION OVER THIS WORK.
2. THE CONTRACTOR SHALL ASSUME THAT THERE WILL BE MINOR MODIFICATIONS MADE DURING
CONSTRUCTION. WORK REQUIRING A SHUTDOWN OF EQUIPMENT SHALL BE AT SUCH TIME AND IN SUCH
MANNER AS DIRECTED BY THE OWNER. PROVIDE A MINIMUM ONE (1) WEEK PRIOR WRITTEN NOTICE.
3. IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO VERIFY FIELD CONDITIONS AT THE SITE AND
NOTIFY THE ENGINEER OF ANY DISCREPANCIES PRIOR TO COMMENCING WITH THE WORK.
4. DURING CONSTRUCTION, THE CONTRACTOR SHALL MAINTAIN A RECORD SET OF INSTALLATION PRINTS.
THE CONTRACTOR SHALL NEATLY AND CLEARLY RECORD ON THESE PRINTS ALL DEVIATIONS FROM THE
CONTRACT DRAWINGS IN SIZES, LOCATIONS AND DETAILS. AT THE COMPLETION OF THE WORK, THE
CONTRACTOR SHALL RETURN THE MARKED PRINTS WITH ALL INFORMATION OBTAINED DURING THE
CONSTRUCTION PROCESS TO THE ENGINEER FOR SUBMISSION TO OWNER.
5. UNLESS OTHERWISE NOTED, ALL MATERIAL AND DESIGN SPECIFICATIONS CITED HEREIN SHALL CONFORM
TO THE MOST RECENTLY ADOPTED SPECIFICATION OF CODE.
6. THE CONTRACTOR SHALL VERIFY AND COORDINATE THE DIMENSIONS AMONG DRAWINGS PRIOR TO
