Frontier - Johnston County MSWLF — Landfill Gas Collection System Expansion 2020 Construction Documentation Report August 2020 Table of Contents Page ii DocuSign Envelope ID: 606B4F05-005F-4133-B694-6E4055737FE7 1.0 OVERVIEW This Construction Documentation Report has been prepared to document the construction of an expansion of the landfill gas (LFG) collection and control system (GCCS) located on the Phase 4A municipal solid waste (MSW) landfill unit of the Johnston County Landfill facility near Smithfield, North Carolina. The landfill facility is owned and operated by Johnston County under State Solid Waste Permit No. 51-03. Frontier Operation Services (Frontier) owns and operates the landfill gas management system under agreement with the County. Original approval for construction of the GCCS was issued by the North Carolina Division of Waste Management (NCDWM) on February 18, 2011 (see Appendix A). 2.0 PROJECT DESCRIPTION 2.1 General The GCCS expansion (layout per DGP) includes the installation of following components: • Seven (7) LFG extraction wells (EW-201 - EW-207) • Approximately 2,050 LF of LFG transmission piping (HDPE (DR 17)): o 8-inch diameter: 1,550 LF; and o 4-inch diameter: 500 LF. 2.2 Reference Documents The project was constructed in accordance with the following documents: Construction Drawings - 2020 GCCS Expansion - Johnston County Landfill: Prepared by Tent Engineering and dated June 26, 2020; Drawings show the location of LFG extraction wells and transmission piping and includes the LFG extraction well schedule. Solid Waste Permit Modification - Landfill Gas Collection and Control System - Johnston County MSW and C&D Landfill Facility - SW Permit #51- 03: Prepared by SCS Engineers, P.C. and dated November 2010 (as revised through January 2011) (Approved by NCDWM on February 18, 2011 (copy of approval letter provided in Appendix M. Photos documenting the construction can be found in Appendix B. Frontier -Johnston County MSWLF - Landfill Gas Collection System Expansion 2020 Construction Documentation Report August 2020 Page 3 DocuSign Envelope ID: 606B4F05-005F-4133-B694-6E4055737FE7 3.1 LFG Extraction Wells Prior to drilling, the location and elevation for each LFG extraction well was surveyed by Jerry Ball Land Surveying. The survey information was verified and approved by Tent Engineering (also reviewed by S+G) to confirm the allowable well depth'. A total of 7 LFG extraction wells having 36-inch diameter bore and a total length and approximate total screened length of 485 feet and 308 feet, respectively, were installed by Kellett's Well Boring, Inc. under contract to Advance One Development, LLC (Advance One) between June 30th and July 7th, 2020. These installations were observed by S+G (Britt Ransom) or S&ME (Ben Best). Spoils generated from drilling operations were inspected by an accredited asbestos inspector in accordance with the Asbestos Work Plan. No suspect asbestos containing materials were observed during this project. Well logs are provided in Appendix C. Once each well was drilled, Kellett's Well Boring and Advance One performed completion activities which were generally as follows (given dimensions are approximate): • 1 foot of No. 3 (non -calcareous) stone was placed in the bottom of the well bore. • The pipe casing (6-inch diameter SCH 80 PVC) was then installed with the perforated section ending between 20-30 feet below the ground surface and solid pipe extending vertically to the surface. • The well was then backfilled with additional No. 3 stone to between 18-28 feet below the ground surface (about 2 feet above the perforated section). • 1 foot of soil backfill was added. • A 2 foot thick expanding foam seal was added. • Soil backfill was added to 5 feet below ground surface. • A 2 foot thick expanding foam seal was added. • Soil backfill was added to the ground surface, compacted, and sloped away from the well casing. • Following well completion, an approximately four (4) foot section of solid casing remained above ground as a stickup. • Each well was fitted with a vertical wellhead assembly and connected to transmission piping. ' The total targeted well depth was determined by taking the difference of the existing ground elevation and the top of protective cover (from prior record drawing information) and deducting a buffer of at least twenty (20) feet. Frontier -Johnston County MSWLF - Landfill Gas Collection System Expansion 2020 Construction Documentation Report August 2020 Page 4 DocuSign Envelope ID: 606B4F05-005F-4133-B694-6E4055737FE7 3.2 LFG Transmission Pioin Advance One installed the HDPE transmission piping (including 4-inch and 8-inch diameter HDPE (DR 17) pipe) during July 2020 and made the corresponding connections to newly installed LFG extraction wells and the pre-existing LFG piping at various locations. At each of two connections with pre-existing header piping a butterfly valve was installed for control of the system. Upon completion of the transmission piping, Advance One performed a pressure test of the piping, which was observed by S&ME. Documentation of the pressure test is provided in Appendix D. Record drawings, prepared by Tent Engineering, showing the location of LFG extraction wells and transmission piping are provided in Appendix E. 4.0 DOCUMENTATION ACTIVITIES In satisfying the requirements of the site's approved CQA Manual, the following activities were performed: Observation and documentation of the installation of LFG extraction wells, system piping, and valves. Review of record drawing. Preparation of this documentation report. 5.o LANDFILL GAS (LFG) COLLECTION G CONTROL SYSTEM MATERIALS S+G observed the LFG extraction well materials installed by Advance One. S+G observed the LFG piping after installation had taken place.
