PURPOSE OF SITE VISIT: Comprehensive Follow Up Inspection STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: NA The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1. The purpose of this site visit was to conduct a follow up inspection. 2. Currently has a small working face, with soil ready for cover and cones/barrels to guide traffic. Entrance road changing as operations continue on the LCID. Side slope along entrance road needs grading to 3:1 slope and cover any exposed waste. Excavation, grading, and fill material side slopes must not exceed a ratio of three horizontal feet to one vertical foot (3:1) according to Rule 15A NCAC 13B .0565(3)(j). Solid waste must be covered with a minimum of six (6)-inch-thick of suitable soil cover or to a thickness to cover exposed wastes, whichever is greater. 3. Be sure and keep trees and large shrubs from growing on top and sides of the LCID in the future. 4. The screening of asphalt material was done on top of northern cell of LCID. Follow up inspection showed the operation has ceased and been removed. Upon inspection the top of the LCID was being covered, graded and sloped. Staff contact Mr. Waters and discussed grading correctly to prevent any storm water from ponding and soaking in to cause leachate but also prevent erosion control. This will also assist in helping establish vegetation, which is necessary next, and it taking root. Please continue to fix according to permit and address any new operational changes during permit renewal. Changes to the operations of this facility shall be submitted for approval from the Section before commencing. Submit for review and approval.
