Operations Plan — Approved August 30, 2016 (DIN 2667) PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The facility is secured by an automatic gate at the entrance of the facility. 2. The required signage is posted at the entrance of the facility to prevent unauthorized access. 3. The facility is open from 8 AM to 4 PM Monday through Friday. 4. The facility is permitted for the treatment of medical waste with steam sterilization, as well as the storage and processing of treated and untreated medical waste. 5. The facility is permitted to use one steam sterilization (autoclave) unit. 6. The operating floor, trailer storage area and unloading docks are cleaned daily. 7. Windblown litter was observed around the compactor which was pointed out to Mr. Weaver. Mr. Weaver stated he would provide photos at the end of the day. Windblown litter around compactor. Photo taken by Daniel Weaver 8/20/2020. 10. Windblown litter is collected at the end of each operational day and was being picked up during the inspection. 11. All trucks are equipped with spill kits. 12. Treated medical waste is loaded into a compactor, then transported to CMS Landfill in Concord for disposal. 13. The facility refrigerated trailer was down during the inspection. Mr. Weaver stated there were several boxes in the trailer along with four pallets in the loading bay area that were scheduled to be transported today by shuttle to Covanta in Atlanta, GA for incineration. 14. All medical waste is processed every other day. 15. All recycled containers are washed, disinfected and inspected for damage prior to being returned to customers. 16. All treatment carts are relined after each autoclave cycle. Records Review - Due to COVID-19 field restrictions the following records were provided via e-mail. 17. Once a week the autoclave is tested to ensure the sterilization is being achieved. Spore test data records (Autoclave Quality Control Log) were reviewed from July 2, 2019 to July 30, 2020. 18. Reviewed RMW (Regulated Medical Waste) Container Logs from July 1, 2019 to August 3, 2020. For the month of July 2020, 14,855 sharps containers were processed and 2,775 RMW containers were processed. 19. Reviewed Container Repair Logs from August 21, 2019 to July 22, 2020. Reviewed the Autoclave Cooks Log for the following dates: April 16, 2020; May 9, 2020 & June 16, 2020. 21. The facility processed 2,540,458 lbs. of medical waste from July 2019 through June 2020.
