Permit to Operate renewed January 30, 2019. Permit to Operate expires June 28, 2023. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None. Long.:-80.181527' The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Page 1 of 5 FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management NORTH CAROLINA Solid Waste Section ADDITIONAL COMMENTS: 1. The facility is a land clearing and inert debris (LCID) landfill. Facility operations include the treatment and processing of used concrete and clean wood waste, both of which are defined in the permit renewal issued January 30, 2019. The permit renewal also includes a 5.56-acre expansion of the landfill disposal area. 2. A new structure that will serve as the facility office had recently been moved onto the site and was being set up at the time of this inspection. Ensure that the following documents are kept at the facility at all times: • Current permit to operate; • Current approved operations plan; • Current approved site plan drawings; • Landfill cover log. 3. Proper signage was observed at the facility entrance. 4. The site is secured by means of a gate that is kept locked when the facility is not open to the public. 5. Since the previous inspection on August 22, 2019, major changes had occurred at the site. The tree line that had separated the landfill (northwest) portion of the site from the road had been removed. Construction of a berm was underway between Old Thomasville Road and the landfill to assist with stormwater management and to prevent unauthorized access to the facility. Some wood waste and metal scrap were observed along the berm. Ensure that all unacceptable material is removed from the berm and the surrounding access roadway and disposed of properly. 6. Trees had also been removed from the area along the western property line and along the southern property line in the proximity of the landfill. 7. During the land clearing process, most of the permanent edge of waste markers had either been removed or knocked down. Ensure that permanent edge of waste markers are reinstalled along the landfill's disposal boundary. Markers must be maintained so that they are upright and visible around the entire perimeter of the landfill. 8. Mr. Phillips verified that operations were still confined to the originally approved 3.32-acre landfill footprint. Prior to moving into the 5.56-acre expansion area, please provide the engineer's construction certification and as -built drawing, and arrange for a pre -operations inspection of the facility by the Solid Waste Section, as required by the permit to construct Pre -Operational Condition #5 — items a through h. 9. The facility was open at the time of this inspection; however, no customers were observed onsite. 10. It was raining during the inspection, and the site was very wet and muddy. Ponding water was visible mainly on the southern half of the property, and the rainwater had created small channels that cut across the site and through internal roadways, flowing toward the sediment basin near the southeast corner of the operating area. 11. The sediment basin appeared to be at capacity. Site conditions prevented the inspection of the outfall, which is located on the south side. Mr. Phillips stated that the pond had been emptied, cleaned out and repaired as requested by the erosion control inspector who had visited the site in October 2018. 12. The access roadway leading to the landfill tipping area is of all-weather construction and made up of compacted gravel and soil. The roadway and tipping area were stable, with sufficient space to allow trucks and heavy equipment to maneuver safely. Other internal roads leading to the inert debris processing area and to the soil stockpiles were muddy with ponding water and appeared to be unsuitable for vehicles and heavy equipment to navigate. 13. The working face of the landfill was small, and the waste appeared to be well -compacted. No water was observed ponding in or around the waste. No unacceptable materials were observed. Mr. Phillips stated that the landfill was due to be covered with clean soil the following week. 14. Only a small excavated area remains in the original 3.32-acre landfill cell. Some erosion was visible on the excavated slope. Please ensure that the excavated slope is maintained at a grade no steeper than 3:1 to prevent erosion. 15. The stockpiles of wood waste that were staged on top of the landfill during the previous inspection on August 22, 2019 had been removed to the disposal area and incorporated into the landfill. Several small piles of logs suitable for milling had been stacked just east of the disposal boundary. Please ensure that all wood waste, including logs, is stockpiled within the approved wood waste operations area located at the northwest corner of the site. Inert debris processing currently takes place near the southeastern end of the original facility operations area. This area will be relocated to a new 1.2-acre area, identified in the 2019 permit renewal, as landfill operations expand. 17. Large stockpiles of crushed concrete, unprocessed concrete, and used asphalt were situated around the processing equipment. Mr. Phillips stated that the frequent rainstorms had made processing operations almost impossible over the past three months. Please ensure that the amount of inert debris stored onsite — both processed and unprocessed — does not exceed the 1,000-ton maximum specified in the permit to operate. 18. A roll -off container for the collection of scrap metal is staged adjacent to the inert debris processing area. The container was observed to be full, and a pile of scrap metal had accumulated on the ground next to it. Mr. Phillips stated that he was awaiting the swap -out of the metals container. Ensure that all metal waste at the facility is collected and stored in the designated roll -off container. 19. A series of metal bars is set into the access roadway just inside the entry gate. Vehicles leaving the site are required to cross the bars so that excess mud and dirt are removed from tires prior to entering the public roadway. No mud was observed on the roadway outside the facility entrance. Logs, flanked by clean soil stockpiles, are stored adjacent to the landfill.
