Life of Site, issued on December 10, 2019 and expires on October 08, 2053. PURPOSE OF SITE VISIT: Partial Facility Compliance and DS08-001 Inspection. STATUS OF PAST NOTED VIOLATIONS: Corrective action is still in process from April 17, 2020 inspection. OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 4 ADDITIONAL COMMENTS: 01 - This inspection was conducted to see progress made on corrective actions from the April 17, 2020 inspection. 02 - Progress has been made on the cover placement as noted on the April 17, 2020 inspection report. Still needs some improvement on repairs on bare areas and gas line installation areas. 03 - The windblown litter issue appears to be resolved during this inspection. 04 - Work is still being conducted on areas which will not have additional wastes placed on them for 12 months or more, but where final termination of disposal operations has not occurred, shall be covered with a minimum of one foot of intermediate cover. 05 - Leachate tank area looks good and tanks have adequate storage available. 06 - Facility burns off landfill gas. The system appears to be working correctly looking at the flares. 07 - Groundwater Monitoring Wells that were inspected, were locked and had tags with some well information still missing. A copy of the well information requirements has been emailed to Matt Einsmann and Michael Kavanugh. 08 - All the roads were maintained and constructed of all-weather material. 09 - Records were not reviewed during this inspection due to the Covid-19. Matt Einsmann and John College discussed some of the records that can be emailed for reviewed for the next inspection. 10 - Due to Heavy rainfall, the South end of the new cell has an area that has eroded with exposed waste. This needs to also be repaired ASAP. 11 - The fencing caught the waste but it needs to be collected and put back into the cell and covered with 6 inches of cover material. 12 - The Disaster Debris Site (DS08-001) area was evaluated for additional acres added to this existing TDDS and to get the GPS coordinates. This evaluation form and map has been submitted and approved. 13 - All COVID-19 CDC Recommendations were followed during this inspection. 14 - Digital pictures were taken of the observed violations and some are attached to this report.
