During the completeness review meeting held on January 29,
1991, you requested supplementary information for Champion's
previously submitted document, Response to Review Comments,
Landfill No. 6, Area A. Enclosed with this letter is a bound
document, in triplicate, entitled Construction Quality
Assurance Plan for Landfill 6 Area A. This CQA Plan replaces
the previous response to Review Comment #4.
We are also submitting four (4) sets of revised construction
drawings incorporating the changes discussed. Please discard
the previous sets cf drawings submitted foY L-Andfi-1 No. 6
Area A.

This office has reviewed the erosion and sedimentation control plan
submitted for the project listed below. We find the plan to be acceptable and
hereby issue this Letter of Approval. Please be advised that Title 15A, North
Carolina Administrative Code 46.0017(a), requires that a copy of the approved
soil erosion control plan be on file at the job site. Also, you should consider
this letter to give the Notice required by NCGS 113A-61(d) of our right of
periodic inspection to ensure compliance with the approved plan.
The State's Sedimentation Pollution Control Program is a performance -
oriented program requiring protection of the natural resources and adjoining
properties. If, following commencement of this project, it is determined that
the plan is inadequate to meet the requirements of NCGS 113A-51 to 66, this
office may require revisions to the plan and implementation of the revisions to
ensure compliance with the Act.
Please note that this approval is based in part on the accuracy of the
information provided in the Financial Responsibility Form which you have
provided. You are requested to file an amended form if there is any change in
the information included on the form. In addition, it would be helpful if you
notify this office of the proposed starting date for this project. Your
cooperation is appreciated.

Amendment to Permit No. 44-06
Dear Ms. Hoyt:
Please find enclosed revised construction drawings in
triplicate, dated June 14, 1991, for the proposed
expansion of Champion's Landfill No. 6, Area A. As we
discussed on June 3, these revised drawings incorporate
three minor changes to improve the overall design of the
cell. Each modification is highlighted on the drawings
as indicated below.
The first modification, highlighted in green, lowers the
elevation of the west end of Celt I to increase cell
capacity and improve operation by creating a more level
cell for easier operation. The resulting changes in
elevation are Indicated on drawings G9083-2, G9083-4,
G9O83-5, and G9083-7.
The second modification, highlighted in yellow, raises
the elevation of the northeast corner of Cell IV to
increase capacity. This change is indicated on drawings
G9O83-2, G9083-3, G9083-5, and G9083-7.
The third modification, highlighted in blue, adds a new
12-inch steel stormwater pipe jacked and bored through
the east end of the Cell IV embankment. In the original
design, the stormwater pipe was to be pushed through the
existing 36-inch pipe for the underdrain. By separating
the stormwater and underdrain pipes, construction and
operation will be simplified. The addition of the new

Pipe and the headwall concrete detail are highlighted on
drawing G9083-2. The removal of the stormwater line from
the junction box is shown on drawing G9083-6, Detail 10.
We are also submitting today the Erosion Control Plan,
one set of revised construction drawings, and the
required permit fee to Mr. Richard Phillips, Land Quality
Section - Asheville Office, for approval.

Dear Ms. Hoyt:
In response to your questions earlier this week regarding
Champion International Corporation's permit application
for the Canton Mill's expansion of Area A, please find
enclosed a copy of the No. 6 Landfill Permit with the
Register of Deeds seal from Haywood County. As we
discussed, Champion plans to completely install the liner
over the 32-acre area in one construction season.
At Champion's request, Sirrine Environmental has provided
the following attachments to provide information
pertaining to the resistance of carbon black stabilized
polyethylene to UV radiation. The attachments consist of
the following:
Attachment 1 - The Durability of HDPE Liner, in
Particular Gundline HD (Gundle). This attachment
describes four different types of resistances
(chemical, ultraviolet, biological and stress crack)
specific to the Gundle Gundline HD liner.
Attachment 2 - UV Resistance of Gundline HD
(Gundle). This attachment describes the carbon
black dispersion necessary for optimum outdoor UV
exposure.
Attachment 3 - The Weatherability of Polyethylene -
Carbon Black Formulations (W. Lincoln Hawkins).
This attachment was written by a materials
consultant and describes the effectiveness of carbon
black as a stabilizer for polyethylene.

Attachment 4 - The Twenty Year Weathering of
Polyethylene Compounds Containing Carbon Black (D.
Malcolm Walker and Marc W. Juzkow). This attachment
contains a long term study on the effects of carbon
black with respect to maintenance of the physical
properties of polyethylene after being exposed to
external weathering for twenty years.
Each attachment has highlighted (underlined) main points
which deal primarily with either carbon black's
resistance to UV radiation or its general resistance to
long-term outdoor exposure. It should be noted that the
carbon black content, as specified in the CQA Plan as
2-3%, and carbon black dispersion, as specified in the
CQA Plan as A-1 or A-2, are suitable 60 mil geomembrane
specifications for optimum UV radiation protection.
Attachment 2 states these carbon black specifications for
outdoor UV exposure.
The Canton Mill has 8 years of operating experience with
three leachate ponds lined with HDPE at the No. 6
Landfill. You may recall seeing these ponds during your
tour of the No. 6 Landfill with George Pickard on August
13, 1991. An estimated 80% of the liner, including
seams, is continuously exposed to UV radiation and
weathering. No degradation or failure of the HDPE liner
has occurred during this time. Based on the published
industry studies attached to this letter and Champion's
operational experience, we have concluded that the carbon
black in the liner will provide adequate protection from
UV radiation until the time of use.

construction of Area A has begun.

SIR: AN INSPECTION OF YOUR LAND DISPOSAL SITE HAS BEEN MADE THIS DATE AND YOU ARE NOTIFIED OF THE VIOLATIONS, IF ANY, MARKED BELOW
WITH A CROSS (X).
(1) PLAN AND PERMIT REQUIREMENTS
— (a) Construction plans shall be approved and followed.
— (b) Specified monitoring and reporting requirements shall be met.
(2) SPREADING AND COMPACTING REQUIREMENTS
— (a) Solid waste shall be restricted into the smallest area feasible.
(b) ;,Solid waste shall be compacted as densely as practical into cells.
(3) COVER REQUIREMENTS
(a) Solid waste shall be covered after each day of operation, with a
compacted layer of at least six inches of suitable cover or as
specified by the division.
(b) Areas which will not have additional wastes placed on them for
12 months or more shall be covered with a minimum of one foot
of intermediate cover.
(c) After final termination of disposal operations at the site or a
major part thereof or upon revocation of a permit, the area shall
be covered with at least two feet of suitable compacted earth.
(4) EROSION CONTROL REQUIREMENTS
(a) Adequate erosion control measures shall be practiced to pre-
vent silt from leaving the site.
(b) Adequate erosion control measures shall be practiced to pre-
vent excessive on -site erosion.
(5) DRAINAGE CONTROL REQUIREMENTS
— (a) Surface water shall be diverted from the operational area.
(b) Surface water shall not be impounded over or in waste.
(c) Completed areas shall be adequately sloped to allow surface
water runoff in a controlled manner.
(6) VEGETATION REQUIREMENTS
(a) Within six months after final termination of disposal operations
at the site on a major part thereof or upon revocation of a
permit, the area shall be stabilized with native grasses.
— (b) Temporary seeding will be utilized as necessary to stabilize the
site.
(7) WATER PROTECTION REQUIREMENTS
(a) The separation distance of four (4) feet between waste and
water table shall be maintained unless otherwise specified by
the division in the permit.
— (b) Solid waste shall not be disposed of in water.
(c) Leachate shall be contained on site or properly treated prior to
discharge. An NPDES permit may be required prior to the dis-
charge of leachate to surface waters.
(8) ACCESS AND SECURITY REQUIREMENTS
(a) The site shall be adequately secured by means of gates, chains,
berms, fences, and other security measures approved by the
division, to prevent unauthorized entry.
(b) An attendant shall be on duty at the site at all times while it is
open for public use to ensure compliance with operational
requirements.
(c) The access road to the site shall be of all-weather construction
and maintained in good condition.
(d) Dust control measures shall be implemented where necessary.
REMARKS:
(9) SIGN REQUIREMENTS
— (a) Signs providing information on dumping procedures, the hours
during which the site is open for public use, the permit number
and other pertinent information shall be posted at the site
entrance.
— (b) Signs shall be posted stating that no hazardous or liquid waste
can be received without written permission from the division.
— (c) Traffic signs or markers shall be provided as necessary to pro-
mote an orderly traffic pattern to and from the discharge area
and to maintain efficient operating conditions.
(10) SAFETY REQUIREMENTS
(a) Open burning of solid waste is prohibited.
— (b) Equipment shall be provided to control accidental fires or
arrangements shall be made with the local fire protection
agency to immediately provide fire -fighting services when
needed.
—(c) Fires that occur at a sanitary landfill shall be reported to the
division within 24 hours and a written notification shall be
submitted within 15 days.
— (d) The removal of solid waste from a sanitary landfill is prohibited
unless the owner/operator approves and the removal is not
performed on the working face.
— (e) Barrels and drums shall not be disposed of unless they are empty
and perforated sufficiently to ensure that no liquid or hazard-
ous waste is contained therein.
(11) WASTE ACCEPTANCE AND DISPOSAL REQUIREMENTS
— (a) A site shall only accept those solid wastes which it is permitted to
receive.
— (b) No hazardous, liquid, or infectious waste shall be accepted or
disposed of in a sanitary landfill except as may be approved by
the division.
(c) Spoiled foods, animal carcasses, abattoir waste, hatchery waste,
and other animal waste delivered to the disposal site shall be
covered immediately.
— (d) Asbestos waste that is packaged in accordance with 40 CFR 61, as
amended through January 1,1985, may be disposed of separate
and apart from other solid wastes at the bottom of the working
face or in an area not contiguous with other disposal areas, in
either case, in virgin soil. Separate areas shall be clearly marked
so that asbestos is not exposed by future land -disturbing activi-
ties. The waste shall be covered immediately with soil in a
manner that will not cause airborne conditions. Copies of 40
CFR 61 may be obtained and inspected at the Solid and Hazard-
ous Waste Management Branch, Division of Health Services,
P. O. Box 2091, Raleigh, NC, 27602.
(12) MISCELLANEOUS REQUIREMENTS
— (a) Effective vector control measures shall be applied to control
flies, rodents, and other insects or vermin when necessary.
(b) Appropriate methods such as fencing and diking shall be pro-
vided within the area to confine solid waste subject to be blown
by the wind. At the conclusion of each day of operation, all
windblown material resulting from the operation shall be col-
lected and returned to the area by the owner or operator.

Dear Mr. Hocutt:
Champion International Corporation is resubmitting a
request for approval to dispose of a process tank residue
in a company owned and operated landfill. Please find
attached the Procedure and Criteria for Waste
Determination form for this material and the Toxicity
Characteristic Leachate Procedure (TCLP) analytical data.
The request previously submitted on September 7, 1990, is
attached for reference.
The waste described was removed from the bottom of the
West Evaporator Feed Tank during a cleaning operation.
The tank residue is fiber, miscellaneous inert materials,
and black liquor solids which were mixed with bark for
disposal.
The Evaporator Feed Tank residue is not listed nor does
it exhibit any of the four hazardous waste
characteristics. It is not ignitable, corrosive (10.53
pH), reactive (stable mixture), nor does it contain any
compounds above the N.C. regulatory limits for the
Toxicity Characteristic Leachate Procedure (TCLP).
Laboratory results from AnalytiKEM and Pace Incorporated
are attached.
The Evaporator Feed Tank Residue would be disposed of in
Area B of Champion International Corporation's No. 6
Landfill, Permit No. 45-06. Area B has leachate
collection, and is a disposal area for sludge and fly ash
waste from the mill. The Evaporator Feed Tank Residue
would be disposed of in accordance with the Solid Waste
Management Rules.

This procedure will be used by the Solid Waste Management Division to determine -
whether a waste is (1) hazardous as defined by 15A NCAC 13B, and (2) suitable for
disposal at a solid waste management facility. The types of wastes that will be
evaluated by this proqedure are primarily, but not exclusively, industrial and
cxxmx=1a1 wastes and sludges, and Publicly Owned Treatment Works Slues.
The Solid Waste Management Division reserves the right to request additional
information or waive some of the r n s based on the type of waste if it deems
necessary. The Division may also require some wastes to be heated or altered to render
the wastes envirmu ntally iile prior to disposal at a sanitary landfill. Wastes
disposed at sanitary landfills must be non -liquid which can be properly managed in
accordance with the 1BSolid Waste Management Rules11. APPROVAL TO DISPOSE OF THE APE

West Black Liquor Evaporator Feed Tank at the Champion
International Corp. Landfill, (Permit #45-06).
Dear Mr. Bonistall:
The Solid Waste Management Division has received a request for
characterization of the subject waste. Based upon the submitted
information, the waste appears to be non -hazardous. The Solid Waste
Management Division has no objection to the sanitary landfill
disposal of this waste, if in accordance with the conditions listed
below:
1. The waste contains no free liquids and can be confined,
compacted, and covered in accordance with the Solid Waste
Management Rules (15A NCAC 13B).
2. Approval for sanitary landfill disposal is explicitly
limited to a "one-time" basis. Further approval by this
office will require submittal of a new Waste Determination
form, and (if applicable), new analytical data.
Failure to meet these conditions may result in revocation of
this approval, and subsequently, an administrative penalty. If you

This procedure will be used by the Solid Waste Management Division to determine
whether a waste is (1) hazardous as defined by 1.5A NCAC 13B, and (2) suitable for
disposal at a solid waste management facility. The types of wastes that will be
evaluated. by this procedure are primarily, but not exclusively, i ylustrial and
eaunercial wastes and sludges, and Publicly Owned Treatment Works Sludges.
The Solid Waste Management Division reserves the right to request additional
information or waive sane of the requirements based on the type of waste if it deems
necessary. The Division may also require some wastes to be treated or altered to render
the wastes environmentally immobile prior to disposal at a sanitary landfill. Wastes
disposed at sanitary landfills must be non -liquid 'which can be properly managed in

Dear Mr. Bonistall:
General Statute 130A-294 authorizes the Solid Waste Management
Division to regulate solid waste management in North Carolina. It,
therefore, gives authority to the Division to evaluate wastes that
may pose a threat to the environment or to public health and to
subsequently prohibit disposal of these wastes at a sanitary landfill
in accordance with Sections .0103(d) and (e) of the Solid Waste
Management Rules, 15A N.C.A.C. 13B.
Your recent request for disposal of the subject waste has been
reviewed and evaluated by section personnel. It is concluded that
the waste is not suitable..:for landfill disposal at the designated
facility because of the established NC policy of not allowing wastes
containing detectable levels of PCBs to be disposed in unlined
sanitary landfills. For this reason, your request is denied. Since
the PCB levels are <50 ppm (4.6 and 18.0) and therefore not regulated
by EPA, it is possible that these wastes can be disposed in a secure
site and not a hazardous site. This may be out-of-state. The only
NC Landfill which could be approved by this office to accept these
wastes is the lined facility in Kernersville (Waste Management's

Zhis procedure will be used by the Solid Waste Management Division to determine
whether a waste is (1) hazardous as defined by ISA NCAC 13B, and (2) suitable for
disposal at a solid waste management facility. The type_, of wastes that will be
evaluated by this procedure are primarily, but not exclusively, industrial and
camemial wastes and sludges, and. Publicly Owned Treatment Works Sludges.
The Solid Waste Management Division reserves the right to request additional
information or waive some of the requirements based on the type of waste if it deems
necessary. The Division may also require some wastes to be treated or altered to render
the wastes environmentally immobile prior to disposal at a sanitary landfill. Wastes
disposed at sanitary landfills must be non -liquid which can be propexly managed in
accordance with the "Solid Waste Management Ries". APPROVAL TO DISPOSE OF THE VaME
