The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The purpose of this inspection to verify the site is in compliance and ready to open.
2. The facility consists of a Notified Yard Waste Facility approved for the collection of yard trash only as described in
the approved Yard Waste Notification — New Facility form.

3. Vegetative waste will be ground on -site, and temporarily stored on -site prior to be moved to the town park for
distribution to the public.
4. The Town of Granite Quarry received a letter on August 11, 2020 from David Miller, PE, State Mining Engineer,
approving the closure of the mining permit for Herman Quarry, which is located on this site. Closure of the quarry
was required in order to operate the yard waste facility.
5. The Town has acquired the necessary approved erosion control plan and a General Construction Stormwater
Discharge permit to convert the site to a notified yard waste site.
6. A gate was located at the entrance to the facility to prevent unauthorized access.
7. The access road is paved into the site with a gravel drive around the utility building.
S. A privacy fence will be installed along the NE side of the paved driveway to the gate.
9. The facility is not open to the general public for disposal of yard trash and is only used by the Town of Granite
Quarry. The town provides curb side collection of yard trash for residents.
10. The site is in compliance with all buffer requirements.
11. The working area of the site is less than 2 acres. It is estimated that the amount of unprocessed and processed waste
will be much less than the 6000 cubic yard maximum permitted for a notified site per 15A NCAC 13B .1402(6)(A).
