The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. This inspection was performed to verify closure of the yard waste facility.
2. During the previous inspection conducted on 10-11-2019, Jason Hord stated the Town of Quarry intends to close
this site and sell the property. Further, the town developed another yard waste site, YWN-80-009, which had been
approved on 11-9-2019.
3. This notified yard waste site does not have any yard waste debris stored on -site, and the site has been restored as
required.
4. The privacy fence located around the front of the property is to be removed by Mr. Hord and installed at the newly
approved YWN-80-009 site.
5. The Yard Waste Notification — Notice of Ceased Operation of a Facility form was submitted to the Section on
8/13/2020. The Section received the notification and approved the closure on 8/14/2020.
