The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Met to discuss the permit modification to include a maximizer & additional tanks. The site will become high
capacity (storage in excess of 25,000 gallons) and as such will require double setbacks. At least 200 feet will be
required from the creek to any of the tanks, maximizer etc. Distance was not fully measured at this time due
to excessive vegetation along the creek. Previous site notes for the initial field pre-op indicated that the edge of
the disposal fields to the creek was greater than 200’ but did not give specifics – the proposed tank location is
currently located between the disposal fields and the creek. A 200 feet setback must be measured and
marked from all tanks & maximizer locations directly to the creek path.
2. Received a basic drawing of the proposed facility – a final site drawing will be submitted in the future. A
SDTF application will need to be submitted for this permit modification (a form is included with this
audit). A leak/pressure test (signed off by a PE) will be required for any buried tanks or lines. Also an
operation manual (explaining the waste process from beginning to end) shall be submitted. The screening
debris from the maximizer shall go to a landfill for disposal and so an authorization from the landfill will
be required.
3. Discussed how spills should be handled and that a berm may be installed around the tank area for additional
protection.
4. At this time no firm other than Beltons’ Septic Tank Service is using the two currently permitted tanks. If this
changes in the future, then an authorization form shall be submitted. Also when this permit modification is
issued then treatment logs will have to be maintained.
