B. EXECUTIVE SUMMARY    1.   Scope of Services    WithersRavenel, Inc. (WR), in association with TA‐10, TA‐11 and TA‐12 for the site, completed  the following approved Scopes of Service:     TA‐10:  o PRM provided technical troubleshooting on two hammer head pneumatic pumps,  removed from RW‐19 and RW‐24, and the Goulds e‐SV air stripper transfer  pump.   It was determined that parts are no longer available for the hammer head  pneumatic pumps; therefore, the pumps will need to be replaced.   It was determined that the pump portion of the Goulds e‐SV and been  damaged beyond repair; therefore, replacement of the pump was  recommended.   TA‐11:  o PRM replaced the pump portion of the Goulds e‐SV air stripper pump.   TA‐12:  o WR installed the Goulds e‐SV air stripper pump on January 30, 2020 and  restarted the remediation system.  o Six O&M tasks were completed between January 30, 2020 and March 26, 2020.  o Collection of monthly influent samples.   Influent samples were collected in January, February , and March 2020.  o Collection of quarterly effluent sample.   An effluent sample was collected in February 2020.  o WR performed minor repairs and ordered replacement bag filters for the 50µm  and 10 µm bag filters.  o Preparation and submittal of the enclosed report.    C. SITE HISTORY AND CHARACTERIZATION    The site is located at 125 Hospital Drive in Spruce Pine, North Carolina, and is currently an active  hospital (Blue Ridge Regional Hospital).  A site location map is provided as Figure 1 and the UST  areas and well field are depicted on Figure 2.       The UST system installation contractor failed to connect the excess fuel return line from  the exterior foundation wall of the boiler room (part of the main hospital building at that  time) to the 20,000‐gallon UST. Excess fuel not used by the boiler discharged to the  subsurface rather than returning to the UST system. According to the NCDENR’s  (currently NCDEQ) Registered Tank Database, the hospital operates a UST system that  includes one 20,000‐gallon heating oil UST (installed May 10, 1966), one 7,000‐gallon  heating oil UST (installed May 7, 1976), and associated piping that provides heating oil to  the hospital’s boiler.  In historical reporting, S&ME, Inc. (S&ME) reported that the 7,000‐ Corrective Action Performance Report  April 2020   Page 3 of 6     The release was discovered in May 1988. From 1988 to 2007, soil and groundwater  assessment activities were conducted to determine the extent of the release to the soil  and groundwater. The groundwater beneath the hospital historically flows to the  northwest toward the North Toe River, along with the free product and impacted  groundwater.     In 1994, a remediation system consisting of a groundwater and free product recovery  system was installed and began operation. Treated water from the remediation system  discharges to an unnamed tributary of Beaver Creek (Class C, Trout water), which flows  to the North Toe River within the French Broad River basin. The pump and treat system  at the site consists of sixteen recovery wells and eleven trench wells (located along the  DOT right‐of‐way) equipped with total fluid pneumatic pumps, associated piping, two oil  water separators, and a 25‐horsepower air compressor. Recovered product is stored in a  2,500‐gallon aboveground storage tank (AST).    Based on a previous receptor survey conducted in 2011, two active drinking water supply wells  are located within 1,000 feet of the source area.  Municipal water was determined to be  available to the area.    Operation of the system ceased in 2012 during the time at which operation and maintenance of  the system was the responsibility of Crawford Environmental Services (Crawford). S&ME  reported that it was their understanding that the system was not in operation during Crawford’s  tenure.    In April 2015, S&ME performed an evaluation of the groundwater treatment system to  determine what necessary repairs and maintenance items needed to be addressed to bring the  system back into operation. The findings of this evaluation was provided to the NCDEQ in the  form of a System Enhancement Recommendation Report dated July 22, 2015, which included air  compressor maintenance, electrical service repairs, the need for new carbon vessels, and repairs  to several recovery well pumps. The recommended repairs were made in August and September  2015.    WR took over site O&M activities in January 2019.    D. REMEDIATION SYSTEM OPERATION    1.   Remediation System Status    During this reporting period, WR conducted system maintenance visits on January 30, 2020,  February 2, 2020, February 20, 2020, March 3, 2020, March 18, 2020, and March 26, 2020.   O&M field forms are included as Appendix A.    2.  General System Operation    The air compressor and electrical controls for the groundwater treatment system are housed  in a small building that is located northwest of the Blue Ridge Regional Hospital. Recovered trench through a 2‐inch PVC pipe to two oil/water separators within the treatment system  structure.  Recovered oil drains by gravity into a 3,000‐gallon oil holding AST outside the  treatment system structure and recovered groundwater drains by gravity into a 500‐gallon  batch tank housed inside the treatment system structure.  The water within the batch tank is  pumped first through two particulate filters containing 50‐micon bag filters (plumbed in  parallel) then into a shallow, three tray aeration stripper. The bottom tray of the air stripper is  transferred by a pump through two particulate filters containing 10‐micro bag filters  (plumbed in parallel) and two 1,000‐pound activated carbon filters (plumbed in series). The  water is discharged to a nearby stormwater pipe and eventually into Beaver Creek northwest  of the treatment system. The effluent discharge map is presented as Figure 3, a flow chart of  the system is presented in Figure 4 and the layout of the system is presented in Figure 5.    3.  Total Gallons of Water Pumped During the Period    Between the dates of January 30, 2020 and March 26, 2020, the subject remediation system  treated approximately 900,180 gallons of water and recovered 201 gallons of free product.  Based on this data, approximately 11.16 gallons per minute (gpm) of groundwater was  extracted.  The remediation system operated 100% of the time between January 30, 2020  and March 26, 2020.  The following summary system faults observed on arrival.    1. None.    4.  System Repairs    a. Activated Carbon Backflushing Activities    WR has completed backflushing activities on both activated carbon vessels during each  O&M visit.  Carbon backpressures have increased during this O&M period.  WR will  continue backflushing efforts during each O&M visit.      b. Bag Filter Change Out    WR changes out the pre‐air stripper and post air stripper bag filters during each O&M visit.    c. Pneumatic Pump Maintenance    WR did not encounter any pump issues during this O&M period.    d.
