OBSERVED VIOLATIONS:
1- .0838(c) Septage treatment and detention facility containers shall be constructed of steel, concrete, plastic, or
fiberglass; and shall be free of evidence of damage or weakness such as holes or cracks that may allow the escape
of septage. On the right side of the tank on the far right end of the facility (when facing the storage tanks) there
appears to be two active seep holes on the side of the tank (two wet stain areas were noted). This tank needs to
be emptied of all waste until the two possible holes are adequately patched to prevent any leakage. If the
tank is not repaired then it shall be permanently removed from service and the permit.
2- .0838(i) The area around tanks shall be free of debris and vegetation to allow for access and inspection for a
distance of 5 feet. The vegetation around the three tanks needs to be cut/removed for a radius of at least 5
feet.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The other two tanks (not including the one noted in the observed violation on page 1) appear to be structurally
sound.
2. The approved setbacks are being met.
3. The required signs are posted and the access road is adequate.
4. No apparent spills or leaks of septage on the ground surface. No apparent vectors or odors noted.
