This permit shall expire 28 May 2021. PURPOSE OF SITE VISIT: Pre-operative Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None applicable OBSERVED VIOLATIONS: There were no violations observed during this Facility Compliance Inspection. Please see the ADDITIONAL COMMENTS section below for details concerning this inspection. ADDITIONAL COMMENTS: 1) This is a Solid Waste Transfer Facility and is permitted to receive Solid Waste as defined in NCGS 130A- 290(a)(4); (14); (15); and NCGS 130A-294 (m). 2) This Facility is permitted to receive Construction & Demolition Waste generated in Brunswick, Carteret, Craven, Duplin, Jones, Lenoir, New Hanover, Onslow, Pamlico, Pender, Pitt, and Wayne Counties. 3) Construction and Demolition Waste must be transported for disposal to the Maysville Construction and Demolition Landfill in Jones County, permit #52-03. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 1607T_INSP_20200624 Page 2 of 3 4) Proper signage including the Facility permit number, accepted waste types, banned or excluded items, and the emergency contact number is posted at the Facility entrance. 5) Erosion control devices are in place and appear to be functioning properly. No off-site erosion was observed during this Facility Compliance Inspection. 6) The Facility is secured by means of a locked gate at the Facility entrance. 7) The Facility access road is of all-weather construction and is being maintained in overall good condition. 8) Construction of the Facility has been completed and appears to be incompliance with the approved construction plan. 9) *This Facility may not begin solid waste management operations until the Permit to Operate has been issued.
