Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1) Pumper vehicle(s) inspected: TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) JT-5397 3C7WRMCL8JG3991993 1,000 (waste), 250 (fresh) 2) The firm name and phone number were on the cab doors on both sides of the truck. The permit number and town name were on toolboxes on both sides of the truck. The letters were three (3) inches or greater in height. The lettering was distinguishable and not blocked from view. The lettering meets the requirements of 15A NCAC 13B .0835(b). See pictures below. 3) The tank is made of steel and affixed to the frame of the truck as per 15A NCAC 13B .0835(a)(1). 4) The valves appeared to be in working order and closed as per 15A NCAC 13B .0835(a)(2). Caps present on ports. 5) Access ports were properly sealed with lids that were closed as per 15A NCAC 13B .0835(a)(3). 6) No leaks noticed as per 15A NCAC 13B .0835(a). 7) Truck in compliance with the Septage Rules. Driver side of pumper vehicle. Passenger side of pumper vehicle.
