Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. The inspection was conducted in Mt. Airy, Surry County. 2. The following firm truck was inspected: Vehicle Identification Number (VIN) NC Tag Number Capacity (gallons) 1HTSDAAN3SH215759 YA146142 2000 3. The tanks appeared to be structurally sound and bolted to the frame. 4. Valves and access ports appeared to be secure. Caps were present on the access ports. 5. The trucks were properly marked in accordance with the lettering requirements. 6. No leaks were observed. 7. The permit application lists five more vehicles that will need an inspection. 8. It was indicated that you want to add a second vehicle to your company roster and change the company name. The addition of a second vehicle will require $250.00 (this in addition to the $550.00 that you have already paid). In addition, an application will need to be submitted to change the name of your company. A copy of the application is attached with this report. After you have completed the name change and paid the extra $250.00, you will need to contact Troy Harrison at 828-296-4701 or troy.harrison@ncdenr.gov to arrange an inspection of your pumper vehicles.
