Dear Mr. McQueen: Ozone Waste Solutions (OWS) operated a facility (Facility) at the above-referenced address to
treat and process regulated and unregulated medical wastes by using an ozonation system under
the Solid Waste Permit No. 4126-MWP-2014 [Document Identification No. (DIN) 22560]. On February 12, 2020, OWS officially informed the Solid Waste Section (SWS) that it would withdraw the permit renewal application and terminate permitted waste operations at the Facility. According to the SWS records, the last load of wastes was treated by the ozonation system and
disposed of on May 18, 2016.
The SWS conducted the facility audit on June 23, 2020 and follow-up inspection on June 30, 2020 to inspect the completion of the facility closure activities. According to the Facility Compliance Inspection Report (FID 1417597), the SWS determines that:
1. The closure activities of medical waste treatment & process at the OWS facility was
substantially completed.
2. The solid waste permit to operate waste treatment at OWS facility, Permit No. 4126-
MWP-2014, was rescinded on February 12, 2020.
3. The SWS shall start the process to release the rule-required financial assurance within a

Pursuant to Rule 15A NCAC 13B. 1204(b)(7), OWS shall maintain all records associated with the waste operations and activities at the Facility including this Closure Letter for at least three
