Per receipt of a UST-3 / Notice of Intent to Permanent Close two unregistered underground storage tanks
located at 10 Puckett Trail, Epsom NC 27537, a facility id # has been assigned to this site and the State
Registration database updated accordingly with the tank information.

As per noted on the Letter of Transmittal from WithersRavenel, the Tank / Product Use was updated as
Empty & NIU before 12/22/1988 and, therefore, these tanks are not Billable.
