The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Excess vegetation is growing up around some of the field posts. Remove the excess vegetation from
around the posts. Field posts appear adequate at this time.
2. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are
being met at this time. No sign of heavy or recent application. No odors noted.
3. The required signs are posted and the access road is adequate.
4. The cropstand appears adequate at this time except for some scattered dead patches. Some of these areas
appear to be grease burns – Mr. Cooper stated that the vegetation had died due to wet conditions.
Repair work will most likely be needed in these areas to reestablish the cropstand. Implement weed
control as needed in the coming months.
5. Soil samples shall be taken by the site operator this fall. Received a new key for the entrance gate.
6. An authorization form was submitted for Hunt’s Septic Cleaning to utilize this site in 2020. If this occurs
then site logs shall be submitted monthly to this office.
