Re: Documentation for Ground -water Monitoring Wells at the
Graham County Sanitary Landfill (Permit #38-01)
Dear Mr. Crisp,
William L. Meyer
Director
Upon a recent review of our files, our office found that we do
not have sufficient documentation on the ground -water monitoring
wells at the Graham County Landfill facility. Although we received
Well Completion Records for the recently installed monitoring wells,
the forms were incomplete. The wells were not numbered on the Well
Completion Record forms and there was no map showing where the wells
are located. Within the next 30 days would you please provide our
office with the following information:
1. A map (preferably 8 1/2" x 11") delineating the entire
landfill and adjacent property owned (or leased) by you.
This map should clearly show the location of the landfill
with respect to the entire property.
2. A specific site map of the landfill itself illustrating the
configuration of the landfill per the original permit
description. On this map also indicate the following:
a. Areas of past landfilling activity
b. Area of present landfilling activity
c. Area of proposed future landfilling activity
d. Areas of demolition waste landfilling
e. Locations of existing monitoring wells; Each well
should be numbered on the map (e.g. MW-1; MW-2; etc.)
in a manner corresponding to the designations of the
wells in the field.
3. Complete and accurate Well Completion Records (DHS 3342) for
all ground -water monitoring wells. These records should
clearly indicate the following:
Page 2
a. Well construction specifications pertaining to
inner/outer casing diameter
b. Borehole depth
C. 24-hour water table elevations
d. Screen length
e. Screen slot size
f. Vertical extent of screen (e.g. where the screen is
placed in the aquifer).
g. Which wells are "upgradient" and which wells are
"downgradient".
h. All other data required on well construction forms as
specified in the "N.C. Water Quality Monitoring
Guidance Document for Solid Waste Facilities".
In addition, please submit to us a summary of the hydro -
geological data accumulated for the landfill site from the boring
operation. Include boring logs and other pertinent subsurface
information. Also, designate in writing to this office the name of
the contractor you have chosen to perform your annual ground and
surface water monitoring extractions, and the date on which you plan
to perform your annual sampling.
If you have questions or comments please contact this office at

Re: Installment of Ground -water Monitoring Wells at the Graham
County Landfill (Permit #38-01)
Dear: Mr. Brooms,
As referenced in the letters from this office dated March 31,
1987 and September 21, 1987, solid waste facilities are required to
construct ground -water monitoring wells and implement a ground and
surface water monitoring program by June 30, 1989. The ground -water
monitoring system should be constructed and implemented in a manner
consistent with the specifications of the "North Carolina Water
Quality Monitoring Guidance Document for Solid Waste Facilities".
Recently the well locations specified in the amendment to your
permit were approved. Therefore, in order to comply with the June
30, 1989 deadline, you should go ahead with well construction.
Within 15 days upon the completion of well installation, a
complete and accurate Well Completion Record (DHS 3342) should be
submitted to this office for all ground -water monitoring wells.
These records should clearly indicate the following:
- Well construction specifications pertaining to inner/outer
casing diameter
- Borehole depth
- 24-hour water table elevations
- Screen length
- Screen slot size
- Vertical extent of screen (e.g. where the screen is placed
in the aquifer).
- All other data required on well construction forms as
specified in the "N.C. Water Quality Monitoring Guidance
Document for Solid Waste Facilities".

In addition, please submit to us a summary of the hydro -
geological data accumulated for the landfill site from the boring
operation. Include boring logs and other pertinent subsurface
information. Also, within 15 days of well installation, designate in
writing to this office the name of the contractor you have chosen to
perform your annual ground and surface water monitoring extractions,
and the date on which you plan to perform your annual sampling.
If you have further questions or comments please contact this
office at (919) 733-0692.

Subject: Disposal of 35-40 yds initially and 15 - 25 yds thereafter every 3 to 6
months of WWTP sludge at the Graham County Landfill, (Permit #38-01).
Dear Mayor Rogers:
The Solid Waste Management Section has received a request for
characterization of the subject waste. Based upon the submitted information, the
waste appears to be non -hazardous. The Solid Waste Management Section has no
objection to the sanitary landfill disposal of this waste, if in accordance with
the conditions listed below:
1. The owner/operator of the landfill approves the disposal.
2. The waste contains no -free liquids.
3. The sludge is used as a soil conditioner and incorporated within the
final two feet of earthen cover at agricultural rates.
4. Additionally, a submittal of the Waste Determination Form and supportive
analytical data, is required on the second anniversary of this approval.
The landfill may place limits on the amount of sludge received based upon
available capacity and weather conditions.
Failure to meet these conditions may result in revocation of this approval,
and subsequently, an administrative penalty. If you have further questions or
comments concerning this matter, please contact this office at (919) 733-0692.

PROCEDURE AND CRITERIA FOR WASTE DETERMINATION
This procedure will be used by the Division of Health Services
to determine whether a waste is (1) hazardous as defined by 10 NCAC
10F, and (2) suitable for disposal at a solid waste management
facility.
The types of wastes that will be evaluated by this procedure are
primarily, but not exclusively, industrial and commercial wastes and
sludges, and Publicly Owned Treatment Works Sludges.
The Division of Health Services reserves the right to request
additional information or waive some of the requirements based on the
type of waste if it deems necessary. The Division may also require
some wastes to be treated or altered to render the wastes
environmentally immobile prior to disposal at a sanitary landfill.
Wastes disposed at sanitary landfills must be non -liquid and in a
form that can be confined, compacted, and covered in accordance with
the "Solid Waste Management Rule". APPROVAL TO DISPOSE OF THE WASTE
SHALL ALSO BE OBTAINED FROM THE OWNER OR OPERATOR OF THE LANDFILL
PRIOR TO DISPOSAL.
The following information is required for an evaluation.
Incomplete waste determination forms will be returned to the
generator without review. An asterisk(*) denotes information
required for Publicly Owned Treatment Works.
