(ATC) is pleased to submit this Federal/State Lead Letter Report documenting assessment activities performed in October 2015 at the Ferrell Residence site located at 5379 Wiggins Mill Road in Lucama, Wilson County, North Carolina. A site topographic map is included as Figure 1 and a site map depicting pertinent features is included as Figure 2. Water supply well information for receptors in the site vicinity are included as Table 1 and shown on Figure 3. Assessment activities were performed based on an August 20, 2015, Task Authorization (TA-4) by the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste Management, North Carolina Department of Environmental Quality (NCDEQ). The work is being performed as part of the State Lead Contract #N14004i between NCDEQ and ATC. On October 15, 2015, ATC supervised the installation of monitoring wells MW-7 and MW-8 located at the former Barnes Store. Regional Probing, Inc. (Regional Probing) installed the monitoring wells using hand auger and Geprobe techniques. The wells were installed to 15 feet below ground surface (bgs) and screened from 2 to 15 feet bgs. Lithology encountered during the monitoring well installation consisted of sandy clay, clay, and sand. Field notes documenting well installation activities are included as Appendix A. Soil boring logs and well construction records are included as Appendix B. Note that monitoring well MW-7 was originally proposed to be installed near former monitoring well MW-5R; however, due to farming activities being performed on the property, the well location was moved closer to the property boundary. ATC mobilized to the site on October 22, 2015, to collect depth to water measurements from all monitoring wells associated with the site and to collect samples from newly installed monitoring wells MW-7 and MW-8. Monitoring wells MW-1 and MW-5R could not be located during the sampling event. The former Barnes store building was recently demolished and monitoring well MW-1 may have been destroyed as a result of the onsite demolition. Well construction information and groundwater elevation data is summarized on Table 2 and a site map indicating monitoring well Federal/State-Lead Letter Report December 2, 2015 Ferrell Residence – Incident # 12611 ATC Project Number SL1261104 2 locations is shown on Figure 2. Field notes from the sampling event are included in Appendix A. Based on the groundwater level measurements collected during the October 2015 sampling event, groundwater generally flows west across the site. A groundwater elevation contour map is included as Figure 4. The groundwater samples were collected in laboratory-supplied, preserved containers; entered into chain of custody control; placed on ice; and shipped to North Carolina-accredited Con-Test Analytical Laboratory in East Longmeadow, Massachusetts. The samples were analyzed by EPA Method 6200B for VOCs including petroleum constituents. The analytical results were compared to the North Carolina Title 15A, NCAC, Subchapter 2L, Part .0202 (2L Standards) for groundwater and Gross Contamination Levels (GCLs). The laboratory results did not indicate any compounds exceeding 2L Standards. Groundwater analytical results are included as Table 3 and historical groundwater analytical results are included as Table 4. A chemicals of concern in groundwater map is included as Figure 5. As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of the laboratory report was conducted. During the review, it was found that the samples were received from the laboratory in acceptable condition and properly preserved. The samples were analyzed within correct holding times and reported within the lab report, and the chain of custody was properly completed by all parties. The laboratory QC report shows that an internal blank was analyzed for each method that was used for the original samples. The laboratory fortified blank (LFB) and laboratory control sample (LCS) data results were also provided for each analytical method. Refer to the laboratory report in Appendix C for details regarding QA results for the laboratory samples and any noted exceptions. Overall, ATC concludes that the laboratory results are usable. Based on the laboratory analytical result, monitoring well MW-1 has indicated impacts to groundwater above the 2L Standards while the remaining wells have not indicated petroleum impacts above standard since August 2010 (monitoring well MW-5R). As a result, the groundwater plume appears stable. Therefore, ATC recommends performing the following tasks: • Perform an updated receptor survey to confirm which properties in the area are connected to a municipal water supply; • Based on a fluctuating groundwater flow direction at the site between monitoring events, ATC recommends re-surveying all monitoring wells associated with the site and collecting groundwater elevation measurements to confirm the groundwater flow direction; • Contact the property owner of the former Barne’s store to determine the future use of the property; and • Perform fate and transport modeling to determine potential impacts to off-site receptors. If the modeling demonstrates the site does not pose a risk to off-site receptors, the site could be a candidate for closure with issuance of a Notice of Residual Petroleum.
