2.0 EXECUTIVE SUMMARY
ATC Associates of North Carolina, P.C. (ATC) has completed a Federal/State-Lead Groundwater
Monitoring Report (GMR) for the Ferrell Residence Site. This report documents well installation
activities and a groundwater sampling event conducted at the site in May 2015. The GMR was
performed based on a March 31, 2015 Task Authorization (TA-3) by the Trust Fund Branch of the
Underground Storage Tank (UST) Section, Division of Waste Management, North Carolina
Department of Environment and Natural Resources (NCDENR). The work is being performed as
part of the State Lead Contract #N14004i between NCDENR and ATC.
The Ferrell Residence is located at 5379 Wiggins Mill Road in Lucama, Wilson County, North
Carolina. The site is currently used for residential purposes and is developed with one single-
story building. The former Barne’s Store is located approximately 270 feet east northeast of the
Ferrell Residence and formerly contained the USTs which impacted the site. Six monitoring wells
(MW-1, MW-2RR, MW-3, MW-4R, MW-5R, and MW-6) are located at the site. Four water
supply wells (WSW-1 through WSW-3 and WSW-6) are located within 1,000 feet of the site.
However, only one water supply well (WSW-6) is currently used for drinking water. The other
three water supply wells are either inactive (WSW-1 or WSW-2) or used for irrigation (WSW-3).
Municipal water is available to the site and surrounding properties.
On May 13, 2015, ATC personnel mobilized to the site to install monitoring wells MW-2RR, MW-
4R and MW-6. On May 19, 2015 ATC personnel mobilized to the site to collect groundwater
samples, measure groundwater elevations and survey the newly installed monitoring wells. In
addition, Oxygen Releasing Compound (ORC) socks were installed in monitoring well MW-1.
Groundwater elevations and samples were collected from monitoring wells MW-1, MW-2RR,
MW-3, MW-4R and MW-6. Monitoring well MW-5R could not be located during this sampling
event.
Groundwater elevations measured in monitoring wells MW-1, MW-2RR, MW-3, MW-4R and
MW-6 indicate that groundwater flows towards MW-1. Laboratory analytical results of the

sampling event indicate petroleum hydrocarbon compounds at concentrations exceeding North
Carolina 2L Groundwater Standards (2L Standards) in monitoring well MW-1.
Due to the reluctance of the water supply well owners to abandon their wells, ATC recommends
performing semi-annual sampling at the site to determine the effectiveness of the ORC socks in
monitoring well MW-1. In addition, ATC recommends the installation of two additional
monitoring wells at the former Barnes’s Store to confirm the groundwater flow direction and
horizontal extent of the contaminant plume. One monitoring well will be installed near MW-5R
(which could not be located during the most recent sampling event and is potentially destroyed)
and to the northwest of monitoring well MW-1. The next sampling event should occur in
November 2015.
3.0 SITE DESCRIPTION
3.1 Site Characterization
The Ferrell Residence site is located approximately 0.31 miles west of the junction of Scott Church
Road and Wiggins Mill Road in Lucama, Wilson County, North Carolina, and is currently
developed with a residential home. The former Barnes Store was located approximately 270 feet
east northeast of the Ferrell Residence and was used as a convenience store and petroleum retail
facility. Two 650-gallon USTs of unknown contents (assumed gasoline or diesel) and one 250-
gallon kerosene UST were located at the former Barnes Store. It is estimated that the USTs were
taken out of service in the early 1960’s; however, the exact year is unknown. The USTs were
removed from the former Barnes’s Store in 1995. The site is surrounded by properties used for
residential or agricultural purposes. Municipal water is available to the site and vicinity.
Seven water supply wells (WSW-1 through WSW-7) are located within 1,500 feet of the site, four
of which are within 1,000 feet of the site. Water supply well ownership information is included in
Table 1. A site topographic map is included as Figure 1 and a site map depicting monitoring well
locations and other pertinent site features is included as Figure 2. A Potential Receptor Map
showing water supply well locations is included as Figure 3.

3.2 Site Background
In August 1994, the NCDENR Division of Environmental Management sampled potable wells
located at the Ferrell Residence (WSW-1) and Deans Residence (WSW-2). Water supply well
WSW-1 water supply well contained 1,2-dichloropropane at 0.39 micrograms per liter (µg/L),
xylenes at 1.2 µg/L, and naphthalene at 15 µg/L. Water supply well WSW-2 contained
chloroform at 0.69 µg/L and 1,2-dichloropropane at 0.59 µg/L.
ATEC Associates (ATEC) submitted an Initial Investigation Report to NCDENR on December
8, 1994. During the investigation, ATEC conducted a site reconnaissance and collected samples
from water supply wells WSW-1 through WSW-3. Based on the laboratory results, chloroform
was detected in WSW-2 at 4.2 µg/L. Based on the results, ATEC recommended UST closure at
the former Barnes Store.
On February 2, 1995, two 650-gallon USTs of unknown contents (although it is assumed they
formerly contained either gasoline or diesel) and one 250-gallon kerosene UST were removed
from the site. Soil samples collected during the UST closure indicated total petroleum
hydrocarbons (TPH) gasoline range organics (GRO) and diesel range organics (DRO) above the
NCDENR Action Level of 10 mg/kg. An Underground Storage Tank Closure Report was
submitted on March 17, 1995.
ATEC submitted at Subsurface Investigation Report on October 30, 1995, documenting the
installation of monitoring wells MW-1 through MW-4 and soil and groundwater sampling. Soil
samples collected from the MW-1 boring indicated a concentration of 88 mg/kg of TPH GRO.
In addition, the groundwater collected from MW-1 indicated concentrations of petroleum
impacts exceeding 2L Standard. Water supply well WSW-2 contained chloroform at a
concentration of 12.2 µg/L.
Mid-Atlantic Associates, P.A. (Mid-Atlantic) submitted a Report of Environmental Services on
October 11, 1996, documenting a comprehensive groundwater sampling event and slug testing
data. Mid-Atlantic performed a geoprobe investigation in December 1996 in order to delineate

the groundwater plume. During the investigation, 13 shallow groundwater samples (GP-1S
through GP-13S) and six samples from the deeper aquifer (GP-3D, GP-4D, GP-5D, GP-11D,
and GP-12D) were collected. The sample collected from GP-10S exhibited concentrations
exceeding 2L Standard. The Geoprobe Investigation Report was submitted on January 29, 1997.
Between March 1997 and March 2002, eight groundwater sampling events were performed.
During a sampling event conducted in April 2000, monitoring well MW-2 could not be located
and was presumed to be destroyed.
A Mobile Multi-Phase Extraction (MMPE) event was performed of the site from June 9, 2003
through June 13, 2003. At the conclusion of the event, approximately 1,724 gallons of
petroleum impacted groundwater were recovered from monitoring well MW-1. A groundwater
sampling event was performed on July 9, 2003, to determine the effectiveness of the MMPE
event. Based on the results of the groundwater sampling event, an additional MMPE event was
performed from December 1, 2003 through December 5, 2003. At the conclusion of the event,
approximately 964 gallons of petroleum impacted water was recovered from MW-1. A sampling
event to monitor the effectiveness of the second MMPE event was performed on January 13,
2004.
A Soil Investigation, Monitoring Well Installation, and Groundwater Monitoring Report was
submitted in March 2005 and documented the advancement of 11 soil borings (SB-1 though SB-
11) at the former Barnes Store property, the installation of MW-2R, and a comprehensive
sampling event. Based on the analytical results, none of the soil borings had indicated petroleum
constituents and monitoring well MW-1 was the only impacted well.
A Soil Removal and Sampling and Groundwater Sampling Report was submitted in June 2006.
During the reporting period, approximately 165 tons of soil were removed from the site.
Following excavation, five soil samples were submitted for analysis of TPH GRO and TPH
DRO. Four of the samples indicated an exceedence of the corresponding Action Level. In
addition, a groundwater sample was collected from temporary well TW-1. Monitoring well

MW-2R could not be located and was determined to be destroyed due to farming activities in the
area.
Between November 6, 2006 and November 1, 2007, three sampling events were performed at the
site. On February 19, 2008, four soil borings were advanced around the surrounding UST area.
Based on the soil analytical results, impacted soil appears to be delineated. In addition, an
updated receptor survey and groundwater sampling event were performed.
On August 19, 2009, monitoring well MW-5R was installed for horizontal delineation of the
groundwater plume. During the September 2009 sampling event, monitoring well MW-4 could
not be located and was presumed destroyed. Between November 19, 2008 and May 19, 2015,
ten groundwater sampling events were performed at the site.
On August 13, 2015, ATC submitted a Federal/State-Lead Monitoring Report documenting the
completion of a sampling event. The laboratory analytical results indicated petroleum hydrocarbon
compounds at concentrations exceeding 2L Standards in monitoring well MW-1. ATC
recommended continued sampling of the existing monitoring well and proximal water supply wells
and the completion of a limited soil assessment.
On January 21, 2015, ATC submitted a Soil Assessment Report documenting the advancement and
collection of soil borings SB-1 through SB-9. Laboratory results indicated no compounds were
detected above Soil-to-Groundwater Maximum Soil Contaminant Concentrations. ATC
recommended installation of monitoring wells in order to delineate the horizontal plume as well as
installation of ORC socks in MW-1.
4.0 MONITORING WELL INSTALLATION
On May 13, 2015, ATC supervised the installation of replacement monitoring well MW-2RR
and MW-4R as well as monitoring well MW-6. Carolina Soil Investigations, Inc. (CSI) installed
the monitoring well using rotary auger techniques. The wells were installed to a depth of 20 feet
below ground surface (bgs) and screened from 5 to 20 feet bgs. Lithology encountered during

the monitoring well installation consisted of brown, fine to medium grained sand. Soil boring
logs and well construction records are included as Appendix A. Field notes documenting well
installation activities are included as Appendix B.
5.0 GROUNDWATER CHARACTERIZATION
5.1 Groundwater Levels
Groundwater level measurements and monitoring well top of casing elevations were collected from
monitoring wells MW-1, MW-2RR, MW-3, MW-4R, and MW-6 on May 19, 2015. Monitoring
well MW-5R could not be located due to recently plowed agricultural land at the site. There is
potential for the monitoring well to be destroyed. ATC surveyed the newly installed monitoring
wells associated with the site using monitoring well MW-1 as a benchmark with an elevation of
100.00 feet. Monitoring well construction details, revised top of casing elevations, and current and
historical groundwater elevation data are summarized in Table 2. Free product was not observed in
any of the monitoring wells that were gauged. Depth to groundwater measurements ranged from
1.96 feet bgs in MW-2RR to 4.29 feet bgs in MW-1. A groundwater elevation contour map
generated using the May 2015 groundwater elevation data is included as Figure 4. This figure
indicates that the direction of groundwater flow at the site is generally towards the former Barnes
Store and MW-1. Based on the topographic map included as Figure 1, groundwater at the site most
likely flows to the south towards the nearest surface water body. Field notes from the May 19, 2015
site visit are included in Appendix B.
5.2 Groundwater Quality
On May 19, 2015, groundwater samples were collected from monitoring wells MW-1, MW-2RR,
MW-3, MW-4R and MW-6. Samples were collected in laboratory-approved containers, packed
in ice and delivered under chain-of-custody protocol to Contest Analytical Laboratory (Contest)
in East Longmeadow, Massachusetts for analyses. Groundwater samples were analyzed for

volatile organic compounds (VOCs) by Environmental Protection Agency (EPA) Method
6200B.
The laboratory analytical results indicated the presence of benzene, ethylbenzene, total xylenes,
naphthalene, isopropylbenzene, n-propylbenzene, 1,3,5-trimethylbenzene and 1,2,4-
thrimethylbenzene at concentrations above 2L Standards in monitoring well MW-1. Compounds
were not detected above 2L Standard in monitoring wells MW-2RR, MW-3, MW-4R, or MW-6.
Current groundwater analytical data are summarized in Table 3 and historical groundwater
analytical data is summarized in Table 4. A chemicals of concern in groundwater map is
included as Figure 5. The laboratory analytical report and chain-of-custody forms are included
in Appendix C.
As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of
the laboratory report was conducted. During the review, it was concluded that samples were
received by the laboratory in acceptable condition and properly preserved. The samples were
analyzed within the correct holding times, and the chain of custody was properly completed by
all parties. The laboratory QC report shows an internal blank was analyzed for each method that
was used for the original samples. The laboratory fortified blank (LFB) and laboratory control
sample (LCS) data results were also provided for each analytical method. Refer to the laboratory
report in Appendix C for details regarding QA results for the laboratory samples and any noted
exceptions. ATC concludes that the laboratory results are usable and any exceptions do not
significantly impact the conclusions presented in this report.
5.3 ORC Sock Installation
On May 19, 2015, ATC personnel installed ORC socks into monitoring well MW-1. The ORC
socks were provided by Regensis in San Clemente, California. The ORC socks are sold in one-foot
sections and are designed to be installed throughout the entire water column. Ten ORC socks were
installed into MW-1 and will remain in place for up to one year to assist in natural attenuation at the
site.

5.4 Water Supply Wells
Four water supply wells (WSW-1 through WSW-3 and WSW-6) are located within 1,000 feet of
the site. Water supply wells WSW-1 and WSW-2 are inactive, WSW-3 is used for irrigation
purposes, and WSW-6 is used for drinking purposes. Table 1 presents the water supply well
ownership information and Figure 3 depicts the water supply well locations. Water supply well
samples were not collected during this sampling event. Historical water supply well analytical
results are included in Table 5.
6.0 CONCLUSIONS AND RECOMMENDATIONS
On behalf of the Trust Fund Branch of the UST Section, ATC has completed a Groundwater
Monitoring Report documenting a well installation and groundwater sampling event in May
2015 for the Ferrell Residence site located at 5379 Wiggins Mill Road in Lucama, Wilson
County, North Carolina. Results from the groundwater monitoring event indicate VOC impact,
including petroleum constituents, to the monitoring well MW-1. No water supply well samples
were collected during this event.
Due to the reluctance of the water supply well owners to abandon their wells, ATC recommends
performing semi-annual sampling at the site to determine the effectiveness of the ORC socks in
monitoring well MW-1. In addition, ATC recommends the installation of two additional
monitoring wells at the former Barnes’s Store to confirm the groundwater flow direction and
horizontal extent of the contaminant plume. One monitoring well will be installed near MW-5R
(which could not be located during the most recent sampling event and is potentially destroyed)
and to the northwest of monitoring well MW-1. The next sampling event should occur in
November 2015.
