Introduction This report documents groundwater sampling activities performed in accordance with the North Carolina State Lead Contract Task Authorization number 13-NCSL09-010-1, authorized on June 12, 2009. The former Four Corners Store is located at the intersection of South Old Carriage Road and West Mount Drive, which is locally referred to as Langley Crossroads, in Nashville, Nash County, North Carolina (Figure 1). The site is currently used as an office for Claude’s Construction, Inc. The area surrounding the site is considered residential, and there is no municipal water available. All drinking water within 1500 feet is supplied by private potable wells. The site meets the criteria for “High Risk” classification because a water supply well used for drinking water is located within 1000 feet of the source area of a confirmed release and the groundwater within 500 feet of the source area of a confirmed release has the potential for future use in that there is no source of water supply other than groundwater. Three water supply wells used for drinking water are located within 500 feet of source area. The closest surface water is a stream 1600 feet west of the site, and the Tar River is approximately one mile southeast of the site. On March 12, 1994, ENSCI Engineering Group, P.A. performed an Underground Storage Tank (UST) Closure event during which two 6000-gallon USTs were removed from the site. The total depth of the excavation of the tank pit was approximately 10 feet below ground surface (bgs), and groundwater was seen entering the excavation. Free product was not identified, but a sheen of petroleum was identified on groundwater located on the south side of the excavation. Analytical results indicated concentrations of total petroleum hydrocarbons above state standards in the area of the former USTs, product lines, and dispensers. According to a Monitoring Well Installation, Groundwater and Water Supply Well Sampling Report prepared by Schnabel Engineering on behalf of Force Environmental Service Company (Force) in July 2006, three monitoring wells (MW-3, MW-4, and MW-5) were installed on May 23, 2006. Monitoring wells MW-1, MW-1D, and MW-2 had been previously installed on February 1, 2006 (Table 1 and Figure 2). Monitoring wells MW-3, MW-4, and MW-5 did not recharge within 24 hours during a Post Installation Sampling Event, so only monitoring wells MW-1, MW-1D, and MW-2 were sampled on May 24, 2006. Groundwater samples from monitoring wells MW-1 and MW-1D contained contaminant concentrations above 15A NCAC 2L .0202 (2L) Standards, while the sample collected from monitoring well MW-2 only contained contaminant concentrations above method detection limits. Water supply wells WSW-1 (onsite), WSW-2, and WSW-3 were also sampled on May 24, 2006 (Table 2 and Figure 3). Water supply well WSW-4 was not sampled because the home was abandoned. None of the groundwater samples from the three water supply wells (WSW-1, WSW-2, and WSW-3) contained contaminant concentrations above method detection limits. Groundwater Monitoring Report – October 5, 2009 NCDENR Incident No. 12610 Four Corners Store Page 4 Nash County, NC A 2008 Groundwater Monitoring Report by Force included results of a groundwater sampling event conducted on May 13, 2008. Six monitoring wells (MW-1, MW-1D, MW-2, MW-3, MW-4, and MW-5) and two water supply wells (WSW-2 and WSW-3) were sampled. Water supply well WSW-1 was not sampled because Force personnel were unable to gain access to a water spigot. This report concluded that monitoring wells MW-1 and MW-5 contained concentrations of petroleum contaminants above the 2L Standards. Contaminant concentrations were not detected above method detection limits in groundwater samples collected from water supply wells WSW-2 or WSW-3. The following report details the results of a groundwater monitoring event conducted by Terraine, Inc. (Terraine) on July 1, 2009. Groundwater Sampling On July 1, 2009, Terraine personnel mobilized to the site to collect groundwater samples from monitoring wells MW-1, MW-1D, MW-2, MW-3, MW-4, and MW-5, and water supply wells WSW-2 and WSW-3 to evaluate current groundwater quality conditions. Monitoring well locations are shown on the site map (Figure 2), and water supply well locations are shown on the aerial site photograph (Figure 3). Prior to sampling, depth to groundwater measurements were obtained from each monitoring well using a decontaminated water level indicator (Table 1). Based on groundwater data collected on July 1, 2009, groundwater appears to flow to the south (Figure 4). After obtaining depth to water measurements, each well was purged in excess of three well volumes and sampled using new disposable bailers. Claude's Construction Company was not open on July 1, 2009. Terraine could not gain access to monitoring wells MW-2 and MW-5; the wells are located behind fencing and the access gates were locked at the time of the site visit. Terraine attempted to contact site representatives; however, no one was available. As a result, monitoring wells MW-2 and MW-5 were not sampled. Monitoring wells MW-1, MW-1D, MW-3, and MW-4 and water supply wells WSW-2 and WSW-3 were sampled. The groundwater samples from water supply wells WSW-2 and WSW-3 were collected from spigots on the back of each house (Table 2). Prior to collecting the samples, water was allowed to run from the spigots for at least 15 minutes to purge the wells and associated piping. All groundwater samples obtained on July 1, 2009 were collected into laboratory prepared containers, placed immediately on ice, and submitted under proper chain of custody control to the Pace Analytical Services, Inc. laboratory in Huntersville, North Carolina, for chemical analyses using Standard Method 6200B for volatile organic compounds, the Massachusetts Department of Environmental Protection method for the determination of volatile petroleum hydrocarbons, and Standard Method 3030C for total lead. Groundwater Sampling Results Table 3 provides a summary of current and historical analytical results. Groundwater monitoring well locations and corresponding analytical data from the July 1, 2009 sampling event are shown in Figure 5. A complete laboratory report and chain of custody form for all groundwater samples obtained July 1, 2009 are included in Appendix C. Groundwater samples collected from the following wells contained concentrations of contaminants above the 2L Standards on July 1, 2009:  MW-1 : benzene; toluene; ethylbenzene; xylenes; 1,2,4-trimethylbenzene; 1,3,5- trimethylbenzene; naphthalene; n-butylbenzene; n-propylbenzene; and C5-C8 aliphatics  MW-3 : lead  MW-4 : lead Groundwater samples collected from the following wells contained concentrations of contaminants above method detection limits but below 2L Standards on July 1, 2009:  MW-1 : C9-C12 aliphatics and C9-C10 aromatics  MW-3 : methyl tert-butyl ether and diisopropyl ether The groundwater sample collected from monitoring well MW-1D contained no concentrations of contaminants above method detection limits. Monitoring wells MW-2 and MW-5 were not sampled due to a locked gate that prevented entry. The groundwater samples collected from water supply wells WSW-2 and WSW-3 contained no concentrations of contaminants above method detection limits. Conclusions Based on the analytical results of the July 1, 2009 groundwater sampling event conducted at the former Four Corners Store site, groundwater collected from monitoring well MW-1 showed the highest contaminant concentrations above the 2L Standards, while groundwater collected from monitoring wells MW-3 and MW-4 only had concentrations of lead above the 2L Standards. Groundwater samples from monitoring well MW-1D and water supply wells WSW-2 and WSW-3 contained no concentrations of contaminants above method detection limits. Topography as well as groundwater elevation data from the four monitoring wells sampled indicate that monitoring wells MW-2 and MW-5 are upgradient of the release source area. B. Recommendations Continued monitoring of the site monitoring wells may help confirm groundwater flow direction and delineate contaminant levels. Terraine recommends the following:  Semi-annual sampling of monitoring wells MW-1, MW-1D, MW-2, MW-3, MW-4, and MW-5 and water supply wells WSW-1, WSW-2, and WSW-3. The next event is proposed for early January 2010. Arrangements will have to be made with the property owner to gain access to the gated monitoring wells MW-2 and MW-5 and an interior spigot to collect a groundwater sample from water supply well WSW-1.  Installing a PRP® Well Boom in monitoring well MW-1. The recommended Well Booms are manufactured by Universal Remediation, Inc. and contain powdered beeswax that stimulates indigenous microbes and promotes rapid bio-degradation of petroleum contaminants.
