In accordance with 130A-294 (a 2), life-of-site ends when the facility reaches its final permitted elevations or by 6 April 2075, whichever occurs first. Any significant site changes may require permit modification. The owner or operator must contact the North Carolina Department of Environmental Quality, Division of Waste Management, Solid Waste Section for coordination of significant site changes, including, but not limited to, pertinent Site, Operation, and Waste Screening plans. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection without records review/pre-construction Facility Compliance Inspection of Phase 2. STATUS OF PAST NOTED VIOLATIONS: None applicable OBSERVED VIOLATIONS: No violations were observed during this Facility Compliance Inspection; please see the ADDITIONAL COMMENTS section below. ADDITIONAL COMMENTS: 1) This Facility is a Construction and Demolition (C&D) waste Landfill and a Temporary Disaster Debris FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 5203_INSP_20200514 Page 2 of 3 Staging Site (TDDSS). 2) This Facility is permitted to receive Construction and Demolition (C&D) waste as defined in NCGS 130A- 290(a)(4) generated in Carteret, Craven, Duplin, Jones, and Onslow Counties. 3) This Facility is currently operating in Phase 1. 4) This Facility is currently receiving C&D waste from the Maysville C&D Recycling and Recovery Facility, permit #52-02. 5) This Facility does not accept wastes containing asbestos. 6) The Facility is secured by means of a locked gate at the Facility entrance. The gate was open upon my arrival and Green Recycling Solutions Personnel were on-site during this Facility Compliance Inspection. 7) The Facility access road is of all-weather construction and is being well-maintained at this time. 8) Facility monitoring wells were reviewed during this Facility Compliance Inspection. All reviewed wells, were readily accessible, secured with functioning locks, and properly labeled. 9) Permanent, highly visible, edge-of-liner markers delineating the waste disposal limits are maintained as required. 10) Soil is stockpiled in an area adjacent to the landfill for use as weekly cover. 11) Windblown debris is being well managed at this time. No windblown debris was observed leaving the Facility during this Facility Compliance Inspection. 12) The waste type was reviewed; only Construction & Demolition waste was observed and no unpermitted waste streams were noted in the active disposal area during this Facility Compliance Inspection. 13) The active areas of the landfill are in need of additional cover. *Please ensure that all waste is being covered with a minimum of six inches of soil at least once per week or when the active disposal area reaches ½ acre in size, whichever occurs first. 14) The operator is in the process of regrading the outside slopes of Phase 1. *Please continue working to ensure all outside slopes are properly covered and graded. Upon completion, please seed and mulch all outside slopes to establish a cover crop suitable to prevent erosion of your landfill cap system. Please monitor the cover crop to ensure that it is established and maintained as required. Any areas where germination does not occur should be re-seeded and mulched promptly. 15) The leachate collection system and storage tanks were reviewed. The system appears to be functioning properly and is well maintained at this time. 16) Groundwater monitoring well #5 is located within the Phase 2 expansion area. *This well should be abandoned in accordance with 15A NCAC 02C and all pertinent abandonment records submitted to the Solid FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 5203_INSP_20200514 Page 3 of 3 Waste Section before construction of Phase 2 begins. 17) No off site erosion was observed during this Facility Compliance Inspection. 18) *Corrective measures are required as a result of this Facility Compliance Inspection.
