The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The permitted site is currently inactive. Measured setbacks (from houses) for a proposed area (previously
permitted) on the north side of the existing site. Received a soil analysis report & recommendation from
Forestry Services for this proposed area. Also received a new SLAS application & nutrient management plan
for the new permit.
2. Posted orange flags on the 500 foot setback radius for this proposed area and also on the upper side where it
runs adjacent to the currently permitted area of Field 1. Instructed Dale Jones to replace the orange flags
with permanent posts. Also discussed that the driving lanes for this new area need to be cleared.
3. This new area will be identified as Field 3 and was GPS’d at this time. Due to the coronavirus situation the GPS
data cannot be finalized and a new complete site map cannot be generated at this time. A basic site map will be
used for the new permit and the GPS data & site specifics will be reviewed in the coming months for a
more accurate site map.
4. The required signs are posted and the access roads are accurate.
5. No land application is occurring on Fields 1 or 2 at this time.
